| 目的 分析平湖地区正常孕妇妊娠甲状腺激素水平的变化，建立早、中、晚期甲状腺功能参考值范围。方法 对786
例不同孕期的妊娠妇女（均符合制定参考范围入选标准）和150 例健康非妊娠妇女，采用化学发光免疫法检测TSH、TT3、TT4、FT3、FT4水平。结果 孕妇的甲状腺激素水平与非妊娠妇女比较差异有统计学意义（P＜0.05），各孕期之间亦存在统计学差异。早、中、晚孕期及非妊娠期TSH 的均数分别为（1.69±1.18）、（1.74±0.70）、（2.46±1.05）、（2.17±0.88）mU/L；TT3 的均数依次为（1.93±0.37）、（2.20±0.38）、（2.02±0.37）、（1.54±0.22）nmol/L；TT4 的均数依次为（108.85±24.43）、（131.60±20.95）、（110.38±23.05）、（89.83±12.48）nmol/L；FT3 的均数依次为（4.67±0.74）、（4.40±0.88）、（4.39±0.67）、（4.36±0.54）pmol/L；FT4 的均数依次为（15.00±0.18）、（13.30±0.18）、（12.40±0.18）和（13.50±0.14）pmol/L。结论 孕妇的甲状腺激素水平不同于非妊娠妇女，因此建立本地区正常孕妇不同孕期的甲状腺激素参考值范围非常有意义。 |

| Objective To establish the trimester-specific ranges of thyroid hormone in pregnant women in Pinghu area of Zhejiang province. Methods Total 786 normal pregnant women without thyroid diseases and 150 normal non-pregnant women(controls) were included in the study. The serum levels of total T3 (TT3), total T4 (TT4), free T3 (FT3), free T4 (FT4), thyrotropin hormone (TSH) were measured with chemiluminescent immunoassay. The reference ranges of thyroid hormones were calculated
as median (the 50th percentile value) and two-sided limits (the 2.5th and 97.5th percentile values). Results The thyroid hormone levels during pregnancy were significantly declined compared with those in non-pregnant women (P＜0.05). There were significant differences in all thyroid hormones among pregnancy women with different trimesters. The median values of serum TSH during the 1st, 2nd and 3rd trimester were (1.69±1.18)U/L, (1.74±0.70)U/L and (2.46±1.05)mU/L, while that was (2.17±
0.88)mU/L in non-pregnant women. The median values of TT3 were (1.93±0.37)nmol/L, (2.20±0.38)nmol/L, (2.02±0.37)nmol/L and (1.54±0.22)nmol/L, respectivley. The median values of TT4 were (108.85±24.43)nmol/L, (131.60±20.95)nmol/L, (110.38±23.05)nmol/L and (89.83±12.48)nmol/L, respectively. The median values of FT3 were (4.67±0.74)pmol/L, (4.40±0.88)pmol/L,(4.39±0.67)pmol/L and (4.36±0.54)pmol/L, respectively. The median values of FT4 were (15.00±0.18)pmol/L, (13.30±0.18)pmol/L, (12.40±0.18)pmol/L and (13.50±0.14)pmol/L, respectively. Conclusion The thyroid hormone levels during pregnancy are different from those of non-pregnant women, so to establish trimester-specific reference ranges of thyroid hormones during normal pregnancy is of importance for clinical practice. |
