IL-33、ST2在慢性阻塞性肺疾病急性加重患者中的表达及意义

摘要:
目的 探讨IL-33、ST2在慢性阻塞性肺疾病(COPD)急性加重患者中的表达及其意义。方法 选取2010年12月至2012年12月COPD急性加重患者30例为病例组,匹配同期健康体检者30例为对照组。使用ELISA法测定治疗前后血清、痰液中IL-33和ST2的表达,并收集患者住院期间肺功能、血气分析、血常规、C反应蛋白(CRP)等临床相关资料进行分析。结果病例组治疗前血清ST2水平显著高于对照组,差异有统计学意义(P<0.05);而痰液ST2、痰液IL-33以及血清IL-33在两组比较中均无统计学差异(均P>0.05)。治疗后病例组血清ST2水平显著低于治疗前(P<0.05),与对照组比较无统计学差异(P>0.05)。病例组治疗后IC%显著性升高(P<0.05),同时CRP显著降低P<0.05)。相关性分析发现血清ST2与IC%、FEV1%呈显著负相关(均P<0.05),与CRP、二氧化碳分压(PCO2)、氧分压(PO2)、乳酸(LA)及血白细胞(WBC)未发现有明显相关性(均P>0.05)。结论血清ST2水平在COPD急性加重期明显增高,治疗缓解后显著降低,并可恢复正常。血清ST2可作为判断COPD病情变化的一项血清生物标志物。

Abstract:
Objective To investigate serum and sputum levels of IL-33 and ST2 in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD) and their clinical significance. Methods Thirty patients with AECOPD (case group) and 30 age-matched healthy subjects (control group) were recruited in this study. The serum and sputum IL-33, ST2 lev- els were measured by ELISA in 30 AECOPD before and after treatment, and 30 healthy age-matched subjects, Results The serum levels of ST2 in AECOPD before treatment were significantly higher than those in normal controls (P<0.05) but the serum levels of IL-33 and the sputum levels of IL-33 and ST2 had no difference with those in normal controls (P >0.05). There were no statistical differences in the serum levels of ST2 between AECOPD patients after treatment and normal controls (P >0.05); and there were significant differences in AECOPD patients before and after treatment (P<0.05).The serum levels of ST2 were nega- tively correlated with FEV1% and IC% in patients(P<0.05). Conclusion Serum levels of ST2 might be used as a marker to evalu- ate the condition of AECOPD patients.
