单侧睾丸扭转对精子DNA 碎片及核蛋白组型影响的研究

摘要:
目的 探讨单侧睾丸扭转以及不同术式对患者精子质量的影响。方法 将58例单侧睾丸扭转患者(其中行睾丸扭转复位术32例,睾丸切除术26例),按WHO《人类精液及精子-宫颈黏液相互作用实验室检验手册(第5版)》规定检测精子运动指标(10项)、精子DNA 碎片和精子核蛋白组型。结果 睾丸扭转复位组术后3、12、26 周的精子密度、前向运动精子比例逐渐升高,差异有统计学意义(P<0.05);精子DNA 碎片百分率、精子核蛋白组型百分率逐渐降低,差异均有统计学意义(均P<0.05)。睾丸扭转复位组术后3周精子DNA 碎片百分率明显高于睾丸切除组,差异有统计学意义(P<0.05);术后26周精子密度明显低于睾丸切除组,差异也有统计学意义(P<0.05)。结论 睾丸扭转使患者精子质量明显降低,单侧睾丸扭转切除术后精子质量的恢复比睾丸扭转复位快。

Abstract:
Objective To investigate the improvement of sperm quality in patients with unilateral testicular torsion after treatmentn. Methods Fifty eight patients with unilateral testicular torsion underwent orchioperxy (32 cases) or orchiectomy (26cases). The sperm concentration, sperm motion indexes were measured, and DNA fragmentation and sperm nucleoprotein transition were detected according to the WHO guidelines. Results Orchiopexy group achieved a significant increase in sperm concentration (P<0.05) and spermprogressive motility(P<0.05) at 3rd and 12th and 26th week after operation; and dramatic decreases in DNA fragmentation and nucleoprotein transition (P<0.05). Compared with orchiectomy group, orchiopexy group showed significantly increased sperm DNA fragmentation at 3rd week after surgery; and remarkably decreased sperm concentration at 26th week after surgery. Conclusion Testicular torsion has a significant negative impact on the sperm quality. Compared with orchiopexy, patients with orchiectomy show a faster recovery of spermquality.
