目的 探讨趋化性细胞受体因子4(CXCR4)、血管内皮生长因子-C(VEGF-C)和基质金属蛋白酶-9(MMP-9)在
胃癌淋巴结转移中的作用及意义。方法 应用免疫组化法检测76例人胃癌组织中CXCR4、VEGF-C 和MMP-9 的表达水平,并分析其与胃癌淋巴结转移的关系。结果 CXCR4、VEGF-C 或MMP-9 任一指标表达水平与胃癌淋巴结转移均有显著关系(均P<0.01),且这3 项指标两两相关,CXCR4/VEGF-C 联合阳性表达的患者淋巴结转移率为84.8%(28/33),高于仅1 项指标阳性表达(11/22,50.0%)及两者联合阴性表达(7/21,33.3%)的患者,差异有统计学意义(均P<0.01);VEGF-C/MMP-9 联合阳性表达的患者淋巴结转移率为80.0%(32/40),高于仅1 项指标阳性表达(9/19,47.4%)及两者联合阴性表达(5/17,29.4%)的患者,差异均有统计学意义(P<0.05 或0.01);CXCR4/MMP-9 联合阳性表达的患者淋巴结转移率为83.3%(30/36),高于仅1 项指标阳性表达(11/22,50.0%)及两者联合阴性表达(5/18,27.8%)的患者,差异均具有统计学意义(均P<0.01)。3 项指标共同表达的患者淋巴结转移率高达90.0%。结论 CXCR4、VEGF-C 和MMP-9 在胃癌组织中的表达与淋巴结转移关系密切,如果3项指标均为阳性者淋巴结转移的可能性最大。 |

Objective To investigated the association of CXCR4, VEGF-C, MMP-9 expression with lymphatic metastasis in gastric cancer. Methods The expressions of CXCR4, VEGF-C and MMP-9 were detected by immuohistochemistry in 76 samples of gastric cancer. The relationship between these markers and lymph node metastasis of gastric cancer was analyzed.Results The expression of CXCR4、VEGF-C and MMP-9 was significantly associated with lymph node metastasis in gastric cancer(P<0.01). The expressions of three proteins were significant correlated with each other. Rate of lymph node metastasis
in patients with expression of both CXCR4 and VEGF-C was 84.5%(28/33), which was significantly higher than that in patients with single protein expression (11/22, 50.0%) or in those with negative expression(7/21, 33.3%). Similarly, the rate in patients with both VEGF-C and MMP-9 expression was 80.0% (32/40), which was higher than that in patients with single protein positive
expression (9/19, 47.4%), or in those with negative expression (5/17, 29.4%). The rate in patients with both CXCR4 and MMP-9 expression was 83.3% (30/36), which was higher than that in patients with single protein positive expression (11/22, 50.0%), or in those with negative expression (5/18, 27.8%). The differences were all found to be statistically significant (P<0.01). The rate of
lymph node metastasis in patients with expression of all three proteins was 90.0%(27/30). Conclusion The expression of CXCR4,VEGF-C and MMP-9 is closely correlated with lymph node metastasis in gastric cancer, patients with positive expression of threemarkers are most likely to have lymph node metastasis. |
