目的对ICU耐药基因广谱β-内酰胺酶(ESBLs)阳性细菌的分布进行监测和干预,从而降低患者感染的风险。方法采集ICU室内空气、医疗设备表面、室内环境表面以及医护人员手指上的样本,以ESBLs基因引物为模板,通过PCR快速检测方式,或抗生素纸片法对混合菌进行监测,同时比较消毒干预前后ESBLs阳性细菌的检出率。结果PCR快速检测方法对耐药基因ESBLs阳性细菌的检测灵敏度显著高于CLSI推荐的纸片法,ICU室内环境、医疗器械和医护人员手指表面分布有ESBLs基因的耐药菌,消毒后检出率显著降低。结论ICU室内空气、环境中各物体表面及医务人员手指表面均可检出耐药基因ESBLs,医护人员应该加强ICU的消毒和监测工作。 |

Abstract: |
Objective To detect extended-spectrum β-lactamase (ESBLs)-producing bacterial contamination in intensive care unit (ICU). Methods Microbial samples were collected from air, surfaces of medical device, environment, fingers and hands of medical personnel in ICU. The ESBLs gene was identified by polymerase chain reaction (PCR), or monitored with antibiotic discs according to CLSI guidelines. The detection rate of ESBLs was compared before and after disinfection. Results The detection sensitivity of ESBLs by PCR was higher than that by antibiotic discs. The ESBLs-producing bacterial strains were detected in the air, medical device, environment and fingers of doctors and nurses before disinfection and not detected after disinfection. Conclusion There is contamination of bacterial strains with ESBLs in the ICU, it is necessary to reinforce the disinfection process and to monitor the ESBLs-producing bacterial contamination in ICU. |
