甲状腺核素99TcmO4-显像在GD甲亢131I疗效评价中的应用研究

摘要:
目的探讨甲状腺核素99TcmO4-显像在GD甲亢131I疗效评价中的应用价值。方法回顾性分析行131I治疗的GD甲亢患者80例,于治疗前、治疗后3个月行血清甲状腺激素、促甲状腺激素(TSH)水平检测及甲状腺核素99TcmO4-显像。根据患者治疗后甲亢症状及体征,血清甲状腺激素、TSH水平将患者分为完全缓解组、部分缓解组、甲减组及无效组,分析比较各组治疗前后甲状腺摄锝率、甲状腺重量的变化差异。结果除无效组外,完全缓解组、部分缓解组和甲减组患者治疗后甲状腺摄锝率、甲状腺重量与治疗前比较差异均有统计学意义(均P<0.05),即完全缓解组、部分缓解组和甲减组患者治疗后甲状腺摄锝率均小于治疗前,甲状腺重量均较治疗前减轻。结论甲状腺核素99TcmO4-显像对甲状腺激素及TSH水平评价GD甲亢131I治疗后疗效具有重要补充作用,临床或可将甲状腺核素显像作为疗效评价手段之一。

Abstract:
Objective To assess the application of 99Tcm-Pertechnetate thyroid scintigraphy in efficacy evaluation of 131-iodine therapy for Graves disease (GD). Methods The clinical data of 80 patients with GD hyperthyroidism treated with 131-iodine were retrospectively analyzed. The serum levels of thyroid hormone and TSH were measured, 99Tcm-Pertechnetate thyroid scintigraphy was performed before and after 131-iodine therapy. After 3 months of 131-iodine treatment, patients were classified as complete remission, partial remission, hypothyroidism and ineffective groups according to the signs and symptoms of hyperthyroidism, and the levels of thyroid hormone and TSH. Thyroid technetium uptake rate and thyroid mass changes were analyzed in each group. Results Three months after 131-iodine treatment the thyroid technetium uptake rate and the thyroid mass of complete remission, partial remission and hypothyroidism groups were significantly different from those before treatmen (P<0.05). Conclusion 99Tcm-Pertechnetate thyroid scintigraphy can be used for efficacy evaluation of GD hyperthyroidism after 131-iodine treatment as supplement to thyroid hormone and TSH levels.
