联合心型脂肪酸结合蛋白及左心耳内径测定在心房颤动近期发生急性缺血性脑卒中患者中的意义

摘要:
目的探讨联合测定心型脂肪酸结合蛋白(heart-typefattyacidbindingprotein,H-FABP)及左心耳内径大小在心房颤动发生急性缺血性脑卒中患者中的意义。方法回顾性分析2015年3至9月住院的心房颤动患者82例,按有无脑卒中分为观察组(心房颤动伴卒中患者)42例及对照组(心房颤动非卒中患者)40例。应用双向测流免疫法测定入院后90min内及6h时的血浆H-FABP浓度,并经食管彩色多普勒超声心动仪,测定左心耳舒张末期内径及收缩末期内径。分析比较两组患者该两项指标。结果入院后90min血浆H-FABP值观察组(12.3±8.2)ng/ml,高于对照组(9.2±4.3)ng/ml(t=2.13,P<0.05),6h时血浆H-FABP值观察组(15.1±8.7)ng/ml,高于对照组(9.8±5.6)ng/ml(t=2.13,P<0.01);左心耳收缩末期内径观察组(19.12±5.78)mm,大于对照组(13.26±4.61)mm(t=2.93,P<0.01),舒张末期内径观察组(25.18±10.42)mm,大于对照组(17.93±9.49)mm(t=3.29,P<0.01)。结论对于心房颤动患者,H-FABP检测及左心耳内径测定对近期发生急性缺血性脑卒中的早期诊断和预后判断具有的一定的意义。

Abstract:
Objective To assess the combined measurement of plasma heart-type fatty acid binding protein (H-FABP) and left atrial appendage diameter in prediction of acute stroke in patients with atrial fibrillation. Methods Clinical data of 82 patients with atrial fibrillation, including 40 complicated with stroke (study group) and 42 without stroke(control group), admitted in Huzhou Central Hospital from March to September 2015 were retrospectively analyzed. Plasma H-FABP levels were measured by bi-directional lateral flow immunoassay at 90min and 6h after admission, and the left atrial appendage end-systole diameter and end-diastole diameter were determined by transesophageal echocardiography. Results The plasma H-FABP level in study group was higher than that in control group (at 90min: 12.3±8.2 vs 9.2±4.3ng/ml, at 6h: 15.1±8.7vs9.8±5.6ng/ml, both P<0.05), and left atrial appendage diameter in study group was larger than that in control group (end-systole: 19.12±5.78 vs 13.26±4.61mm, end-diastole: 25.18±10.42 vs 17.93±9.49mm, both P<0.05). Conclusion Combined measurement of plasma heart-type fatty acid binding protein and left atrial appendage diameter may be used for prediction of acute stroke in patients with atrial fibrillation.
