| 摘要: | 
			 
		     | 目的探讨脑梗死患者恢复期缺血侧脑血流CT灌注、外周血血管内皮生长因子(VEGF)和血管活性肠肽(VIP)水平的变化及临床意义。方法选择脑梗死患者60例,进行药物治疗及康复治疗,测量并比较治疗前、治疗后3、6个月时患者的神经功能评分[美国国立卫生研究院卒中量表(NIHSS)评分、改良Rankin量表(MRS)评分、日常生活活动能力量表(ADL)评分]、CT灌注成像指标[脑血流量(CBF)、脑血容量(CBV)、平均通过时间(MTT)]以及外周血细胞因子水平(VEGF、VIP)。结果脑梗死患者治疗后3、6个月NIHSS评分[(18.56±4.88)、(13.59±4.27)分]和MRS评分[(3.82±0.96)、(2.87±0.81)分]较治疗前[(20.89±5.18)、(4.16±0.79)分]降低,ADL评分[(28.96±5.98)、(39.28±6.81)分]较治疗前[(25.41±6.17)分]升高,CBF、CBV及VEGF水平[(21.34±4.02)ml·100g-1·min-1、(2.24±0.48)ml/100g、(406.28±58.22)ng/ml,(24.89±5.33)ml·100g-1·min-1、(2.49±0.37)ml/100g、(420.44±71.54)ng/ml]升高,MTT、VIP水平[(5.92±1.20)s、(194.25±18.04)pg/ml,(4.99±1.19)s、(182.06±19.08)pg/ml]降低,差异均具有统计学意义(均P<0.05)。结论检测缺血侧脑血流CT灌注情况以及外周血VEGF和VIP水平,对脑梗死患者恢复期肢体功能恢复情况的判定具有一定的参考价值。 |

                | Objective   To investigate the cerebral blood flow and serum levels of vascular endothelial growth factor
(VEGF) and vasoactive intestinal peptide(VIP) in convalescent patients with cerebral infarction. Methods Sixty patients with cerebral infarction were enrolled in the study. The neurological  function  was  evaluated  with  National  Institutes  of Health Stroke Scale (NIHSS), the Modified Rankin Scale (MRS) and the Daily Living Activity Scale (ADL), cerebral CT scan was performed and the cerebral blood flow(CBF), cerebral blood volume(CBV), mean transit time(MTT) were documented; and serum VEGF and VIP levels were measured before and 3, 6 months after treatment. Results Three and 6 months after treatment, the NIHSS scores [(18.56 ±4.88) points, (13.59 ±4.27) points] and MRS scores [(3.82 ±0.96) points, (2.87 ±
0.81) points] were decreased significantly than those before treatment [(20.89±5.18) points, (4.16±0.79) points], while the ADL scores[(28.96±5.98) points, (39.28±6.81) points] were significantly increased than that before treatment [(25.41±6.17) points], the CBF [(21.34 ±4.02)ml•100g-1•min-1, (24.89±5.33)ml•100g-1•min-1], CBV [(2.24±0.48)ml/100g, (2.49±0.37) ml/100g] and VEGF [(406.28 ±58.22)ng/ml, (420.44 ±71.54)ng/ml] were significantly increased; while MTT [(5.92 ±1.20)s, (4.99 ±1.19)s] and VIP [(194.25 ±18.04)pg/ml, (182.06 ±19.08)pg/ml] were significantly decreased(all P<0.05). Conclusion
CT scan of cerebral blood flow at ischemic side and measurement of serum VEGF, VIP levels are of certain reference value for neurological function in cerebral infarction patients during  convalescence. |
