自体腓骨加反“入”字空心螺纹钉双支撑术治疗股骨头坏死临床疗效观察

摘要:
目的 探讨自体腓骨加反“入”字空心螺纹钉双支撑术治疗股骨头坏死的疗效。方法 35 例(48 髋)股骨头缺血性坏死患者,按Ficat病变分期:Ⅰ期8 髋、Ⅱ期31 髋、Ⅲ期(塌陷<3mm)9 髋,采用股骨头髓心减压自体腓骨加反“入”字空心螺纹钉双支撑术,并辅以功能康复锻炼及中药治疗。结果 35例患者平均随访28.7个月。疗效优29 髋,良14 髋,可3 髋,差2 髋,优良率为89.58%。治疗后Harris 评分明显高于治疗前(P<0.05),具有统计学意义。结论 股骨头髓心减压自体腓骨加反“入”字空心螺纹钉双支撑术治疗早期股骨头缺血性坏死,具有手术创伤小,术后功能影响小以及恢复快等优点,是值得选择的临床治疗方法。

Abstract:
Objective To evaluate the clinical effect of autologous fibula supporting technique combined with λ shape double hollow screws in treatment of femoral head osteonecrosis. Methods Thirty five patients (48 hips) with osteonecrosis of femoral head were included in the study. According to Ficat system: 8 hips were in stageⅠ, 31 hips in stageⅡ, 9 hips in stageⅢ (collapse<3mm) . All patients were treated with core decompression of femoral head and autologous fibula supporting technique combined with λ shape double hollow screws. Patients also received functional rehabilitation exercises and traditional Chinese medicine. Results Thirty five patients were followed up for an average of 28.7 months, the clinical outcomes were excellent in 29 hips, good in 14 hips, fair in 3 hips and poor in 2 hips with an excellent and good rate of 89.58%. The Harris score after treatment was significantly higher than that before treatment (P<0.05). Conclusion Core decompression of femoral head and autologous fibula supporting technique combined with λ shape double hollow screws is effective for osteonecrosis of femoral head in early stage, with minimal operational trauma, less postoperative functional limitation and short recovery time.
