降钙素原动态监测在小儿社区获得性肺炎治疗中的价值

摘要:
目的评价降钙素原(PCT)动态监测在小儿社区获得性肺炎(CAP)治疗中的临床价值。方法选取2013年7月1日至2014年12月31日收治的CAP患儿120例,随机分为两组,其中对照组60例,根据儿童CAP管理指南经验性使用抗生素进行治疗;监测组60例,采用血清PCT动态检测法指导抗生素使用,比较两组患儿抗生素使用率、使用时间、住院时间和临床疗效以及监测组患儿血清PCT水平的变化。结果监测组患儿抗生素的使用率较对照组显著下降(63.3%比96.7%,P<0.01),使用时间明显缩短[(4.4±0.5)d比(7.2±0.7)d,P<0.01],且住院时间也显著缩短[(5.5±0.6)d比(8.1±0.5)d,P<0.01],但两组患儿的临床疗效比较差异无统计学意义(P>0.05),在治疗期间均未出现严重并发症;监测组患儿在使用抗生素治疗3d后PCT水平较入院时显著降低(P<0.01)。结论动态监测PCT指导小儿CAP治疗,可明显降低抗生素使用率和使用时间,同时缩短住院时间,具有较好的临床应用价值。

Abstract:
Objective To assess the dynamic monitoring of serum procalcitonin (PCT) levels in guidance of antibiotics use for children with community acquired pneumonia (CAP). Methods One hundred and twenty children with CAP admitted in hospital from July 2013 to December 2014 were enrolled in the study. Patients were randomly divided into two groups: 60 patients in control group received conventional antibiotics treatment according to CAP guidelines for the management of children; 60 patients in monitoring group received antibiotics treatment according to the guidance of dynamic monitoring of serum PCT levels. The rate and duration of antibiotics use, length of hospital stay and clinical efficacy were compared between two groups. Results The rate and duration of antibiotic use in monitoring group were significantly lower than those in control group (63.3% vs 96.7%, P<0.01; 4.4+0.5d vs 7.2+0.7d, P<0.01); the length of hospital stay was significantly shorter (5.5+0.6d vs 8.1+0.5d, P<0.01); there were no significant differences in clinical efficacy between two groups (P >0.05); and there were no severe complications in both groups. Compared to levels at admission, serum PCT level was significantly decreased 3d after antibiotic treatment in monitoring group (P<0.01). Conclusion Dynamic monitoring of serum PCT levels can effectively guide the antibiotic use for children with CAP.
