血清蛋白联合检测在预测妊娠期高血压综合征中的价值

摘要:
目的探讨孕中期孕妇血清蛋白联合检测在预测妊娠期高血压综合征(妊高征)中的价值。方法选取行产前检查并分娩的183例孕妇,根据随访结果将其分为正常对照组94例及妊高征组89例。采用毛细管血清蛋白电泳技术筛查孕中期孕妇血清蛋白(Alb、α1、α2、β1、β2、γ),对血清蛋白与部分妊娠结局的相关性进行分析,应用ROC曲线及约登指数评判各血清蛋白预测妊高征的价值并确定相应的预测界值。结果正常对照组血清Alb水平为(53.68±1.63)g/L,妊高征组为(51.47±1.46)g/L,妊高征组明显低于正常对照组,差异有统计学意义(P<0.01);正常对照组血清α1水平为(5.57±0.51)g/L,妊高征组为(6.17±0.65)g/L,妊高征组明显高于正常对照组,差异有统计学意义(P<0.05);两组间其余血清蛋白比较差异均无统计学意义(均P>0.05)。孕妇血清Alb水平与新生儿出生体重及分娩孕周呈正相关(P=0.011、0.048);血清α1水平与新生儿出生体重及新生儿1minApgar评分呈负相关(P=0.019、0.023)。将血清Alb及α1水平作为检验变量得出预测妊高征的界值,分别为Alb50.02g/L及α16.81g/L。以Alb及α1的联合预测因子作为检验变量绘制ROC曲线并计算约登指数,当联合预测因子为0.038时约登指数最大,为0.62,其预测妊高征的价值最大。结论血清Alb与α1无论是单项还是联合检测,对妊高征发病均有预测价值,但联合检测的价值要优于任一单项检测。

Measurement of serum protein levels in prediction of pregnancy-induced hypertension

Abstract:
Objective To assess the value of serum protein measurement in prediction of pregnancy-induced hypertension (PIH). Methods One hundred and eighty three pregnant women receiving antenatal care at Huzhou Maternity and Child Care Hospital between June 2013 and June 2014 were included in the study, including 94 normal puerperas and 89 with PIH. Serum proteins Alb, α1, α2, β1, β2 and γ were measured at the second-trimester. The correlation between the serological protein levels and outcomes was analyzed with receiver operating characteristic (ROC)and Youden index. Results The mean values of Alb in the control group and PIH group were (53.68±1.63)g/L and (51.47±1.46)g/L, respectively (P<0.01). The mean values of α1 in the control group and PIH group were (5.57±0.51)g/L and (6.17±0.65)g/L, respectively (P<0.05). The Alb level was positively correlated with newborn weight and gestational weeks at delivery (P=0.011 and 0.048). Meanwhile, α1 was negatively correlated with the newborn weight and Apgar scores (P=0.019 and 0.023). The cut-off values of Alb and α1 for PIH prediction were 50.02 g/L and α1 6.81 g/L. The Youden index of the combination of Alb and α1 reached the peak value (0.62) with the joint predictive factor of 0.038. Conclusion Serum protein Alb and α1 levels or their combination may have prediction value for pregnancy-induced hypertension.
