个体化护理改善鼻咽癌调强放射治疗后张口困难的临床研究

摘要:
目的比较个体化护理与常规护理对改善鼻咽癌患者调强放射治疗(IMRT)后的张口困难程度的影响。方法经病理检查确诊为鼻咽癌的患者83例,按随机数字表法分为个体化护理组(42例)和常规护理组(41例),指导患者进行张口训练,定期随访并测量患者的门齿间距。结果两组患者IMRT后2年生存率分别为90.5%、85.4%,差异无统计学意义(P>0.05)。个体化护理组在IMRT后6个月较常规护理组有改善张口困难程度的趋势,但两组比较无统计学差异(P>0.05),12个月后和24个月后差异有统计学意义(均P<0.05)。亚组分析两组IMRT6个月后张口困难Ⅱ级(门齿间距<2cm)患者门齿间距显示个体化护理组较常规护理组在24个月后时张口困难的程度明显改善(P<0.05)。结论个体化护理可明显改善鼻咽癌患者IMRT后的张口困难程度,提高放疗后的生活质量。

Abstract:
Objective To assess the efficacy of individualized nursing care (INC) on the difficulty in opening mouth of patients with nasopharyngeal carcinoma (NPC) treated with intensity-modulated radiotherapy (IMRT). Methods Eighty-three patients with NPC were randomly assigned in two groups, 42 patients received INC and 41 patients received convention nursing care (control group) to train patients in mouth opening, and the distance of up and low incisors were measured during the follow-up. Results There was no difference in 2-year overall survival rate (90.5% vs 85.4%, P >0.05) between two groups. Both groups showed improvement of difficulty in opening mouth, there was no significant difference in the improvement 6 months after IMRT; however, the INC group showed more marked improvement than control group at 12 and 24 months after radiotherapy. Subgroup analysis showed that for patients with grade-II difficulty at 6 months, INC significantly improved the difficulty of opening mouth at 24 months after radiotherapy (P<0.05). Conclusion Individualized nursing care can improve the symptom of difficulty in opening mouth and quality of life for NPC patients treated with IMRT.
