| 目的 探讨妊娠合并甲状腺功能减退症（甲减）患者经左旋甲状腺素钠片治疗后对妊娠结局及子代智力、发育的影 响。 方法 选择建卡、孕检并分娩的 50 例妊娠合并甲减患者作为观察组，均给予左甲状腺素钠片治疗，同时选择同期甲状腺功能正常 孕妇 50 例作为对照组。比较两组孕妇并发贫血、妊娠期高血压疾病及早产情况，以及胎儿宫内窘迫、低体重儿、畸形和死胎情况，并观察 比较婴幼儿 12 个月及 24 个月的智力和发育情况。 结果 观察组合并贫血和早产发生率均高于对照组（均 P＜0.05），而合并高血压疾 病比例略高于对照组，但无统计学差异（P ＞0.05），而两组间胎儿宫内窘迫发生率、低体重儿的百分率以及 12、24 个月的智力指数和 24 个月的发育指数间均无统计学差异（均 P ＞0.05）。 结论 重视妊娠前和妊娠早期的甲状腺功能筛查，及时发现甲减并予甲状腺素治 疗，可大大降低不良妊娠结局的风险及对子代智力、发育不良的影响。 |

| Objective To assess the pregnancy outcome and offspring intelligence development in women with hypothyroidism during pregnancy treated with levothyroxine sodium tablets. Methods Fifty hypothyroidism gravidas receiving levothyroxine treatment during pregnancy (study group) and 50 normal pregnancy women (control group) were recruited. The pregnancy outcome and offspring intelligence development of two groups were analyzed. Results The incidence of anemia and premature delivery in study group (44.0%) was higher than that in control group (12.0%, P＜0.05). There were no significant differences in occurrence of pregnant hypertension, premature delivery, fetal distress, low birth weight, intelligence index and 24 month developmental index between two groups (P ＞0.05). Conclusion The occurrence of adverse pregnancy outcome and adverse offspring intelligence development can be reduced significantly if hypothyroidism can be early discovered and early treated with levothyroxine sodium tablets. |
