目的探讨血清和肽素、sTREM-1及C-反应蛋白(CRP)在慢性阻塞性疾病(COPD)各期中的意义。方法选取AECOPD患者60例为观察组,根据疾病预后分为康复组49例及未康复组11例。分别于入院时、出院前1d(或病情恶化时)检测和肽素、血清可溶性髓系细胞触发受体-1(sTREM-1)及C反应蛋白(CRP)值。其中康复组中有30例患者出院1个月后再次检测和肽素、sTREM-1及CRP值。选择同期健康对照组30例,于清晨空腹抽取静脉血5ml检测和肽素、sTREM-1及CRP值。对比分析3项指标的变化及临床意义。结果康复组和肽素、CRP入院时较出院前及出院后1个月升高(P<0.01),sTREM-1在入院时较出院后1个月升高(P<0.01)。观察组和肽素、CRP及sTREM-1入院时均较对照组升高(P<0.01)。未康复组和肽素水平在恶化时较入院时升高、入院时较康复组升高(P<0.05或0.01)。康复组、未康复组sTREM-1、CRP水平入院时差异均无统计学意义(均P>0.05)。ROCAUC为sTREM-10.729,和肽素0.809,CRP0.960。sTREM-1、和肽素与住院时间呈线性相关,差异均有统计学意义(r=0.256、0.541,P<0.05或0.01)。结论和肽素、sTREM-1、CRP对AECOPD的监测有重要意义,和肽素可以作为病情恶化及住院时间的预测指标。 |

The value of serum copeptin, sTREM-1 and CRP levels in patients with different stages of COPD |

Objective To assess serum copeptin, soluble triggering receptor expressed on myeloid cell-l (sTREM-1), C-reactive protein (CRP) levels in patients with chronic obstructive pulmonary disease (COPD). Methods Sixty patients with COPD were divided into recovery group (n=49) and exacerbation group (n=11) according to the prognosis. Serum levels of sTREM-1, CRP and copeptin were measured at the first day of admission and the day before discharge, and were also measured at d30 after discharge in 30 patients of recovery group. Results Serum copeptin and CRP levels in recovery group at d1 of admission were significantly higher than those at the day before discharge and d30 after discharge (P<0.01). Serum level of sTREM-1, CRP and copeptin in COPD patients at d1 of admission were significantly higher than those in control group (P<0.01). The copeptin level in exacerbate group at disease exacerbated was significant higher than that at admission and also significantly higher than that in recovery group (P <0.05 or 0.01). There were no significant differences in serum levels of sTREM-1 and CRP at d1 between the recovery group and exacerbate group(P >0.05). The ROC curve of sTREM-1, copeptin and CRP was 0.729, 0.809, 0.960 repectively. The levels of sTREM-1 and copeptin had linear regression with hospitalization days(r=0.256、0.541,P<0.05 or 0.01). Conclusion The serum sTREM-1, CRP and copeptin has early predictive value in COPD patients, and copeptin has a predicting value for the disease exacerbation and hospitalization day of COPD patients. |
