| 目的探讨经脐单孔腹腔镜胆囊切除术的安全性、可行性及临床价值。方法选取同一手术团队医师完成的择期行经脐单孔腹腔镜胆囊切除术患者52例（单孔组）及行传统三孔腹腔镜胆囊切除术患者50例（三孔组），比较两组患者手术时间、术中失血量、术后镇痛药物使用情况、术后住院时间及住院费用。结果两组患者均顺利完成手术。单孔组患者手术时间明显长于三孔组[（79.32±40.30）minvs（52.13±15.10）min，P＜0.05]，而术后镇痛药物使用（21.15%vs46.00%）、术后住院时间[（1.25±0.48）dvs（3.42±1.21）d]及住院费用[（1.11±0.06）万元vs（1.32±0.24）万元]比较，单孔组明显优于三孔组（均P＜0.05），且切口美容效果更佳；两组患者术中失血量比较无统计学差异[（27.33±4.12）mlvs（26.41±2.36）ml，P＞0.05]。结论经脐单孔腹腔镜胆囊切除术是安全可行的，具有微创及美容的优势，值得进一步临床推广。 |

| Objective To compare the safety and efficacy of single-port transumbilical laparoscopic cholecystectomy with traditional tree-port approach. Methods Total 102 patients underwent scheduled laparoscopic cholecystectomy by the same surgical team, the single-port transumbilical approach was used in 52 cases (single-port group) and the traditional three-port approach was used in 50 cases (three-port group). The operation time, intraoperative blood loss, length of postoperative hospital stay, analgesic drug use and medical care costs were compared between two groups. Results All operations were successfully performed in two groups. The single-port group showed significantly longer operation time than the three port group [(79.32 ± 40.30)min vs (52.13±15.10)min, P＜0.05], but had better cosmetic appearance. The rate of postoperative analgesic drug use in single-port group was lower than that in three-port group(21.15% vs 46.00%), the length of postoperative hospitalization stay was short [(1.25±0.48)d vs (3.42 ±1.21)d], and medical-care costs [(11.1±0.6) thousand Yuan vs (13.2 ±2.4) thousand Yuan] was less (all P＜0.05). There was no significant difference in intraoperative blood loss between two groups (P ＞0.05). Conclusion Transumbilical single-port approach is feasible, safe, less invasive and of better cosmetic effect for laparoscopic cholecystectomy. |
