2型糖尿病合并非酒精性脂肪肝患者脂联素基因多态性与CT肝/脾比值的相关性研究

摘要:
目的探讨2型糖尿病(T2DM)合并非酒精性脂肪肝(NAFLD)患者的脂联素基因(apM1)多态性与CT肝/脾比值的关系。方法运用PCR直接测序法检测184例无亲缘关系宁波地区汉族人(T2DM合并NAFLD者95例,健康体检者89例)的脂联素基因+45T/G与+276G/T的多态性,并用逻辑回归法分析基因多态性分布与CT肝/脾比值及胰岛素抵抗指数(IR)程度、血糖之间的关系。结果T2DM合并NAFLD患者中,脂联素基因+45和+276位点有T/G单核苷酸多态性(SNP)存在。+45位点的T/G等位基因分布与健康体检者差异有统计学意义(P<0.05),基因型T/G与G/G的突变型人群T2DM合并NAFLD易感性增加(OR=1.947,95%CI=1.082~3.503),+45位点基因突变与患病人群CT肝/脾比值呈负相关(P<0.05),与BMI、稳态胰岛素评价指数(HOMA-IR)、糖化血红蛋白(HbA1c)均无关(均P>0.05)。apM1+276位点亦存在SNP,但与健康人群相比无统计学差异(P>0.05)。结论脂联素基因+45位点T/G、G/G基因突变型人群T2DM合并NAFLD易感性增加,且随着患者的CT肝/脾比值下降,该人群中突变基因型分布增多。

Association between adiponectin gene polymorphisms and liver-spleen CT value in type 2 diabetic patients with non-alcoholic fatty liver disease

Abstract:
Objective To investigate the association between adiponectin gene polymorphisms with liver-spleen CT value in type 2 diabetic (T2DM) patients with non-alcoholic fatty liver disease (NAFLD). Methods PCR and direct sequencing were performed to identify the polymorphism of adiponectin gene in 95 patients with T2DM with NAFLD (study group) and 89 healthy subjects (control group) in Ningbo area. The association of adiponectin gene polymorphism at +45T/G and +276G/T with liver-spleen CT value was analyzed by logistic regression. Results The +45T/G and +276G/T polymorphism in adiponectin gene was detected. Compared to subjects with T/T, the subject with T/G and G/G at SNP45 had higher prevalence of T2DM with NAFLD (OR=1.947, 95%CI=1.082~3.503) and lower liver-spleen CT value [B=-3.231, Exp (B)=0.040]. Polymorphism at +45T/G were not related to BMI, WHR, HOMA-IR, HbA1c (P>0.05). There was no significantly difference in SNP at apM1+276 between experimental group and control group (P >0.05). Conclusion People with T/G and G/G at apM1+45 may have higher susceptibility to T2DM with NAFLD. The frequency of T/G and G/G at apM1+45 is increased as the liver-spleen CT value is decreased.
