目的探讨乳腺导管内癌(DCIS)患者的血脂代谢特点,分析血脂与分子分型、组织学分级及Ki-67的关系。方法选择DCIS患者48例(DCIS组),另择同期浸润性导管癌(浸润组)和良性乳腺疾病(良性组)各150例,所有患者于术前1周内抽取静脉血行血脂检测并进行分析比较,根据术后病理及免疫组化对DCIS行分子分型(LuminalA型和B型、Her-2过表达型、三阴型)和组织学分级(高、中、低分化),观察不同分型、分级患者血脂的表达特点,筛选出与分型有关的血脂指标,并分析其与Ki-67表达水平、组织学分级的关系。结果DCIS组血脂水平与良性组、浸润组比较差异均无统计学意义(均P>0.05)。分子分型后,TC、HDL-C、LDL-C及ApoB水平在不同亚型DCIS患者间差异有统计学意义(均P<0.05)。非Luminal型DCIS患者的TC、LDL-C及ApoB水平高于良性组(均P<0.05),HDL-C低于良性组(P<0.05);Luminal型DCIS患者的TC、HDL-C、LDL-C及ApoB水平与良性组比较差异均无统计学意义(均P>0.05)。DCIS患者TC、LDL-C及ApoB水平与Ki-67水平呈正相关(r=0.5116、0.7582、0.2959,P<0.05),HDL-C水平与Ki-67水平呈负相关(r=-0.4905,P<0.01)。LDL-C水平在不同组织学分级DCIS患者间的差异有统计学意义(P<0.01),组织学分级越高,LDL-C的水平越高。结论不同分子亚型DCIS患者的血脂代谢不同,且血脂水平与分子分型、Ki-67表达水平、组织学分级有关,提示血脂水平可能是DCIS预后的新指标。 |

Objective To investigate the relationsip of serum lipid levels with molecular classification, histological grade and Ki-67 expression in patients with breast ductal carcinoma in situ (DCIS). Methods Forty eight cases of breast DCIS (DCIS group), 150 cases of breast invasive ductal carcinoma (invasive group) and 150 cases of benign breast diseases (benign group) were enrolled in the study. The serum lipid levels were measured one week before surgery in all patients. The molecular subtypes (Luminal A, Luminal B, Over-expression of Her-2 and Triple-negative) and histological grades were classified in DCIS group by histopathological examination and immmunohistochemistry. The association of serum lipids with molecular classification, histological grades and Ki-67 expression was analyzed. Results There were no significant differences in serum lipids between
DCIS group and benign group (P >0.05), DCIS group and invasive group (P >0.05). There were significant differences in total
cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C) and ApolipoproteinB (ApoB) among DCIS of different molecular subtypes (P<0.05). The levels of TC, LDL-C and ApoB in non-Luminal group were significantly higher, and HDL-C was significantly lower than those in benign group (all P <0.05). There were no significant differences in TC, HDL-C, LDL-C and ApoB between Luminal group and benign group (P >0.05). The levels of TC, LDL-C and ApoB in DCIS were positively correlated with Ki-67 expression (r=0.5116, 0.7582, 0.2959, P<0.05), while the level of HDL-C was negatively correlated with Ki-67 (r=-0.4905, P<0.01). The LDL-C levels were increased with the histological grades increasing from high, intermediate to low differentiation (P<0.01). Conclusion The serum lipid levels are correlated with molecular classifi- cations, Ki-67 expression and histological grades, which suggests that serum lipid metabolism may be a new indication of poor prognosis of breast DCIS. |
