线粒体蛋白表达与原发性胆汁性肝硬化病情进展的相关性

摘要:
目的探讨线粒体蛋白对于原发性胆汁性肝硬化(PBC)病情进展的诊断价值。方法收集120例PBC患者肝组织活检标本,其中60例为PBC初期破坏性胆管炎期,60例为PBC终末期肝硬化期,通过AST和ALT水平、免疫组织化学染色及小胆管与胆管量化分析,探究丙酮酸脱氢酶复合-E2(PDC-E2)、细胞色素C氧化酶亚型I(CCO)、微管相关蛋白轻链3β(LC3)、p62、p16INK4a及p21WAF1/Cip1与PBC病情进展的相关关系。结果PBC终末期小胆管区域的PDC-E2和CCO表达明显高于PBC初期患者,差异均有统计学意义(均P<0.05)。PBC终末期和初期患者胆管区域的PDC-E2、CCO、LC3、p62、p16INK4a及p21WAF1/Cip1表达差异均无统计学意义(均P>0.05)。血清AST和ALT水平与两种胆管的LC3的表达呈正相关(r=0.482、0.492、0.588、0.522,均P<0.05),且与胆管内p16INK4a及p21WAF1/Cip1的表达呈正相关(r=0.440、0.612、0.584、0.472,均P<0.05)。结论小胆管的线粒体蛋白表达的情况也作为评估PBC疾病进展的有效标记物,与转氨酶水平呈现正相关。

Abstract:
Objective To investigate the association between the expression of mitochondrial proteins and the progression of primary biliary cirrhosis (PBC). Methods Biopsy specimens were taken from 120 PBC patients, including 60 cases of early stage and 60 cases of end stage. Serum AST and ALT levels were measured, and the expressions of pyruvate dehydrogenase-E2 (PDC-E2), cytochrome c oxidase isoform I (CCO), microtubule-associated protein light chain 3β (LC3), p62, p16INK4a and p21WAF1/Cip1 in tissue samples were detected by immunohistochemical staining. The correlation between the expression of mitochondrial proteins and the progression of PBC was analyzed. Results The expression of PDC-E2 and CCO in the small bile duct area of end stage PBC was significantly higher than that in the early stage PBC (all P<0.05). However, there were no significant differences in the expression of PDC-E2, CCO, LC3, p62, p16INK4a and p21WAF1/Cip1 in the bile duct between end-stage and early stage PBC (all P >0.05). The serum levels of AST and ALT were positively correlated with the expression of LC3 (r=0.482, 0.492, 0.588, 0.522, all P<0.05) in small bile duct and bile duct, and correlated positively with the expression of p16INK4a and p21WAF1/Cip1 in the bile ducts (r=0.440, 0.612, 0.584, 0.472, all P <0.05). Conclusion The expression of mitochondrial proteins in small bile ducts is positively correlated with serum transaminase levels, which may be an useful biomarker for the disease progression of PBC.
