2041例肝硬化病因及并发症分析

摘要:
目的探讨温岭地区肝硬化病因及并发症的特点。方法收集2005年1月至2014年12月住院的2041例肝硬化患者的临床资料进行回顾性分析。结果乙肝肝硬化1565例(76.68%),酒精性肝硬化233例(11.42%),自身免疫性肝硬化52例(2.55%),乙肝合并酒精性肝硬化63例(3.09%),隐源性肝硬化55例(2.69%)。与2005—2009年相比,2010—2014年乙肝肝硬化所占比例由81.82%下降到72.78%,酒精性肝硬化所占比例由7.95%上升到14.04%,自身免疫性肝硬化所占比例由1.25%上升到3.53%,差异均有统计学意义(均P<0.01)。肝硬化并发症主要有原发性肝癌(22.29%)、自发性腹膜炎(18.47%)、上消化道出血(17.20%)、肝性脑病(6.66%)和肝肾综合征(2.01%)。结论本地区肝硬化病因以乙肝为主,酒精性、自身免疫性肝病所占比例有所上升。原发性肝癌是肝硬化最常见的并发症。

Abstract:
Objective To analyze the causes and complications of patients with liver cirrhosis. Methods Clinical data of 2041 patients with liver cirrhosis admitted in Wenling First People's Hospital from January 2005 to December 2014 were analyzed retrospectively. Results Among 2041 cirrhotic patients, 1565 cases (76.68%) were associated with hepatitis B virus infection, 233 cases(11.42%) with alcoholism, 52 cases(2.55%) with autoimmune liver disease, 63 cases(3.09%) with both hepatitis B virus and alcohol, and 55 cases (2.69%) with cryptogenic cirrhosis. The HBV infection as a cause of cirrhosis was decreased from 81.82% in 2005-2009 to 72.78% in 2010-2014 (P <0.01), while alcoholism increased from 7.95% to 14.04% (P <0.01) and autoimmune liver disease increased from 1.25% to 3.53% (P<0.01). The most common complication of cirrhosis was primary hepatocellular carcinoma (22.29% ), followed by spontaneous peritonitis (18.47% ), upper gastrointestinal bleeding (17.20% ), hepatic encephalopathy (6.66%) and hepatorenal syndrome (2.01% ). Conclusion Hepatitis virus B infection is still the main cause of cirrhosis, and the alcoholism and autoimmune disease trend to increase. HCC is the major complication of liver cirrhosis.
