不同胎龄早产儿血胃泌素和生长抑素水平的研究

摘要:
目的探讨早产儿生后12h和7d内血胃泌素(GAS)和生长抑素(SS)水平的变化。方法选择新生儿科收治的早产儿70例,按胎龄分为≤32周组、33~34周组、35~36周组;选择同期出生的健康足月儿45例为对照组。早产儿组与对照组均于生后12h和7d采集空腹静脉血,行GAS、SS测定。结果4组生后12h和7dGAS和SS水平总体比较差异均有统计学意义(均P<0.05);其中早产儿3组生后12h和7dGAS水平均低于对照组,生后12hSS水平均高于对照组,差异均有统计学意义(均P<0.05);早产儿各组间生后12h和7dGAS水平及生后12hSS水平比较,差异均有统计学意义(均P<0.05);而生后7dSS水平比较差异无统计学意义(P>0.05)。4组生后12hGAS和SS水平与生后7d比较差异均有统计学意义(均P<0.05)。早产儿组喂养不耐受发生率明显高于对照组,差异有统计学意义(P<0.05)。结论新生儿GAS、SS水平与胎龄有关,早产儿生后7d内消化功能低下,考虑与GAS低水平及SS高水平有关,临床应根据胃肠激素水平选择合适的喂养时机和方式。

Serum gastrin and somatostatin levels in premature infants of different gestational age

Abstract:
Objective To examine serum gastrin (GAS) and somatostatin (SS) levels in premature infants of different gestational age. Methods Seventy premature infants admitted to neonatal wards in our hospital from August 2014 to December 2015 were included in this study. There were 15 cases of gestational age≤32w, 25 cases of 33~34w and 30 cases of 35 ~36w among 70 premature infants; and 45 full-term newborns served as control group. Serum GAS and SS levels were measured with radioimmunoassay and ELISA at 12h and 7d after birth, respectively. Results The serum GAS levels of premature infants of different gestational ages was significantly lower than that of control group at 12h and d7 after birth (all P<0.05). The serum SS levels of all premature infants was significantly higher than that of control group at 12h after their birth (all P<0.05), while the SS levels of premature infants of ≤32w and 33~34w ages were also higher than that of control group at 7d after birth (all P<0.05). There were significant differences in serum GAS level at 12h and d7 after birth, and in SS levels at 12h after birth among premature infants of different ages(all P<0.05), while there was no significant difference in SS level at d7 after birth(P >0.05). The incidence of feeding intolerance within 72h after birth in preterm infants was significantly higher than that of control group (P< 0.05). Conclusion Serum GAS and SS levels of newborn are closely related to gestational age. The low digestive function in premature infants within 1 week after born may be related to low GAS level and high SS level, which suggests that the gastrointestinal hormone levels might be used for choosing the time of starting breastfeeding and different feeding mode.
