目的探讨低剂量CT结合迭代靶重建(IR)技术在健康体检筛查早期肺癌中的应用价值。方法选取行肺部低剂量CT检查的健康体检者100例(有结节病变者优先选取),与同期行常规剂量CT检查的普通患者100例进行比较。由2位经验丰富的放射科医师独立进行图像解读与评价,并对结节性质进行定性诊断,再与病理切片进行对照;测量并比较两组图像噪声、CT值、信噪比(SNR)、辐射剂量参数[剂量长度乘积(DLP)、容积剂量指数(CTDIvol)、有效剂量(ED)]。结果低剂量组DLP、CTDIvol、ED均明显低于常规剂量组(均P<0.05)。2位医师对低剂量组、正常剂量组的主观图像质量评价结果一致,均符合诊断要求;两组纵隔窗、肺窗主观图像质量评价结果比较,差异均无统计学意义(均P>0.05)。低剂量组图像噪声、SNR均高于常规剂量组(均P<0.05);两组CT值比较差异无统计学意义(P>0.05)。低剂量组中有肺结节病变者57例,其中确诊为早期肺癌7例,炎性病灶20例,钙化及瘢痕结节30例;CT诊断结果与病理诊断结果相符。结论低剂量CT结合IR技术可在健康体检筛查早期肺癌中推广应用。 |

Objective To evaluate the application of low-dose CT imaging combined with iterative reconstruction (IR) technique in screening for early lung cancer. Methods One hundred subjects receiving health check-up underwent low-dose CT with IR (study group) from December 2014 to December 2015, and 100 subjects underwent conventional dose CT scan in the same period (control group). The CT findings were read and evaluated by two senior radiologists independently. The CT value, SNR and radiation dose parameters DLP, CTDIvol, ED were compared between the two groups. Results There was no
significant difference in the subjective image quality between two groups (P>0.05). The SNR and noise in study group were
higher than those in control group (P<0.01), while there was no significant difference in CT value between two groups (P >0.05). The dose (1.21±0.35)mSv of study group was reduced by 60% compared with the control group (2.97±0.60)mSv. All the images met the diagnostic criteria and no lesions were missed. Fifty-seven pulmonary nodules were detected in total 500 subjects undergoing low-dose chest CT, including 7 cases of early lung cancer diagnosed by both imaging and pathology, 20 cases of
inflammatory lesions, 30 cases of calcification and scar nodules. Conclusion The low-dose CT scan combined with iterative reconstruction technique is feasible to be used for the diagnosis of lung nodules. |
