中老年男性肥胖与雄激素水平低下的关系

摘要:
目的探讨中老年男性肥胖与雄激素水平低下的关系。方法2013年4月至2015年4月随机抽取温岭市太平街道中老年男性260例,根据BMI或腰围(WC)分为体重正常126例、超重109例和肥胖25例,测定并比较3组老年男性的血清总睾酮(TT)、黄体生成素(LH)与性激素结合球蛋白(SHBG)水平,并计算不同睾酮补充治疗标准下各组雄激素缺乏率。结果TT、SHBG、LH水平在正常体重、超重、肥胖组间两两比较,差异均有统计学意义(均P<0.05),即TT、SHBG水平均为正常体重>超重>肥胖,而LH水平为肥胖>正常体重>超重。依据《澳大利亚内分泌协会指南》,正常体重、超重和肥胖组雄激素缺乏率分别为2.4%、10.1%和26.0%;依据《中国男科学疾病诊断治疗指南(2013年版)》,正常体重、超重和肥胖组雄激素缺乏率分别为9.5%、17.4%和68.0%。结论中国中老年男性肥胖程度与其雄激素水平密切相关,BMI、WC可作为预测男性雄激素缺乏的指标。

Relationship between obesity and androgen levels in middle and old-aged males

Abstract:
Objective To evaluate the relations between the levels of androgen and obesity in middle and old-aged men. Methods Two hundred and sixty male residents aged 40-80(57.31±9.53) were enrolled in the study by random sampling from Taiping Community of Wenling City between April 2013 and April 2015. According to body mass index (BMI) and waist circumference (WC), the participants were divided into three different groups: normal group(n=126), overweight group (n=109) and obesity group(n=25). The serum total testosterone (TT), luteinizing hormone (LH) and sex hormone binding globulin (SHBG) were measured. The androgen defective rates were calculated according to Australian and Chinese guidelines for standard testosterone supplement therapy. Results There were significant differences in TT, LH and SHBG levels among three groups (P <0.05) (TT: normal >overweight >obesity, SHBG: normal >overweight > obesity, LH: obesity >normal >overweight). According to Australian Endocrine Society Guideline, the androgen defective rates were 2.4% in normal cases, 10.1% in overweight group and 26.0% in obesity group. According to Chinese guidelines for the diagnosis and treatment of male diseases (2013), the androgen defective rates were 9.5% in normal group, 17.4% in overweight group and 68.0% in obesity group. Conclusion The levels of androgen are closely related with the levels of obesity in Chinese middle and old-aged males. BMI and WC can be used as the predictive indexes of androgen deficiency.
