实时三维超声评价围生期心肌病治疗前后左心室收缩功能变化的临床价值

摘要:
目的分析实时三维超声心动图(RT-3DE)在评价围生期心肌病(PPCM)治疗前后左心室收缩功能变化中的临床价值。方法选取40例PPCM患者作为观察对象,分别在入院7d、接受内科治疗3个月后和12个月后,进行RT-3DE和M型超声检查诊断,评定患者左心室收缩功能变化情况,并在入院7d内对同一患者左心室收缩功能进行重复性对比检验。结果与入院7d时比较,治疗3个月后、12个月后,患者的LVESV、LVEDV显著减少,LVEF显著增加,差异均具有统计学意义(均P<0.05);治疗3个月后和12个月后各指标对比结果发现并无明显统计学差异(均P>0.05);RT-3DE检测结果重复性要优于M型超声检测。结论M型超声检测结果情况与RT-3DE检测结果情况走势一致,但是M型超声重复性不如RT-3DE。RT-3DE可对PPCM治疗前后左心室收缩功能的变化情况进行科学准确评价。

Real-time three-dimensional echocardiography in evaluation of left ventricular systolic function in peripartum cardiomyopathy before and after treatment

Abstract:
Objective To assess the application of real-time three-dimensional echocardiography (RT-3DE) in evaluation of left ventricular systolic function in peripartum cardiomyopathy (PPCM) before and after treatment. Methods Forty infants with PPCM treated in our hospital between October 2012 and August 2016 were included in the study. The left ventricular systolic function was evaluated with RT-3DE and M type ultrasonography at 7 days, 3 months and 12 months after admission. Results Compared with d7, the LVESV and LVEDV of 40 PPCM patients were significantly reduced and LVEF was significantly increased at 3 months and 12 months after treatment (P<0.05), but there were no significant difference in above indexes between3 months and 12 months after treatment (P >0.05). The repeatability of RT-3DE results was better than that of M type ultrasonography. Conclusion RT-3DE can effectively evaluate the changes of left ventricular systolic function in infants with PPCM before and after treatment.
