糖尿病肾病患者肾脏组织中尾加压素Ⅱ及转化生长因子β1的表达及意义

摘要:
目的探讨尾加压素Ⅱ(UⅡ)及转化生长因子茁1(TGF-茁1)在糖尿病肾病肾脏组织中的表达特点及其临床意义。方法选择行肾穿刺病理诊断为糖尿病肾病的患者42例,根据Tervaet糖尿病肾病的肾小球病理分型分为病变较轻组(糖尿病肾病Ⅰ期、Ⅱ期,A组)20例和病变较重组(糖尿病肾病Ⅲ期、Ⅳ期,B组)22例;选择泌尿外科行肾脏手术的患者15例作为对照组(C组)。免疫组织化学法检测肾脏组织中UⅡ、TGF-茁1的表达,并对该两项指标间以及与患者其他实验室检查指标的关系进行相关性分析和回归分析。结果UⅡ在肾小球、肾小管的表达与TGF-茁1的表达水平均呈正相关(r肾小球=0.950,r肾小管=0.963,均P<0.01),UⅡ和TGF-茁1在B组表达水平最高,C组表达水平最低,各组间比较均有统计学意义(均P<0.05);,UⅡ和TGF-茁1与血肌酐、血尿素氮、24h尿蛋白定量、尿微量白蛋白、KatafuChi的半定量慢性化积分、总积分均呈正相关,与eGFR呈负相关。回归分析发现,UⅡ、TGF-茁1在肾小球、肾小管的表达与eGFR、半定量积分相关性最强。结论UⅡ及TGF-茁1在糖尿病肾病肾脏组织中存在高表达,并随着病变的加重表达增加,提示两者可能在糖尿病肾病进展过程中起着重要作用。

Expression of urotensin Ⅱ and TGF-β1 in kidney tissue of patients with diabetic nephropathy and their clinical significance

Abstract:
Objective To investigate the expression of urotensin Ⅱ (U II) and TGF-β1 in kidney tissue of patients with diabetic nephropathy and their clinical significance. Methods 42 People who were diagnosed as diabetic nephropathy by renal biopsy in our hospital from January 2012 to January 2015 were divided into two subgroups based on glomerular pathology classification of Tervaet diabetic nephropathys: subgroup A (n=20) which includes type Ⅰand type Ⅱ, subgroup B (n=22) which includes type Ⅲ and type Ⅳ, whereas subgroup C (n=15) is normal control group. Expression of U Ⅱ, TGF-茁1 in kidney tissue can be detected by immunohistochemical method. Results Urotensin Ⅱ and TGF-β1 are overexpressed in the kidney tissue among patients with diabetic nephropathy, and the expression levels are associated with the disease severity, Indicating that they may play an important role in the development of diabetic nephropathy. Conclusion Expression of UII was positively correlated with the expression of TGF-茁1 in glomerulus (r= 0.950, P<0.01)and renal tubule (r=0.963, P<0.01). The expression of UII and TGF-茁1 in diabetic patients was higher than that in normal controls (P<0.05). In addition, expression of U Ⅱ and TGF-茁1 in glomerulus and renal tubule was positively correlated with serum creatinine, urea nitrogen, 24-h urine protein, microalbminurine and KatafuChi semi-quantitative chronic integral and total integral, and negatively correlated with the level of eGFR. And the correlation between the expression of UⅡ, TGF-茁1 and semi-quantitative time delayed integral of eGFR.
