| 目的探讨联合检测结缔组织生长因子（CTGF）和纤溶酶原激活物抑制物-1（PAI-1）对原发性IgA肾病（PIgAN）患者病情评估的价值。方法ELISA法检测50例PIgAN患者（PIgAN患者组）血清CTGF和PAI-1水平，并根据病理Lee氏分级法将PIgAN患者组分为轻度组（Ⅰ～Ⅲ级）和重度组（Ⅳ～Ⅴ级），采用ROC曲线和曲线下面积（AUC）评价CTGF和PAI-1在PIgAN病情评估中的价值。另设年龄、性别相匹配的30例健康体检者作为对照组。结果PIgAN患者组血清CTGF和PAI-1水平均明显高于对照组（均P＜0.05）。重度组患者血清CTGF和PAI-1水平均明显高于轻度组（均P＜0.05）。血清CTGF和PAI-1单独评估患者病情时，ROC曲线的AUC分别为0.844（95%CI：0.736～0.995，P＜0.05）、0.900（95%CI：0.811～0.988，P＜0.05），而两者联合检测评估病情时，其AUC为0.927（95%CI：0.856～0.998，P＜0.05）。结论联合检测血清CTGF和PAI-1对PIgAN患者病情评估有较高价值。 |

| Objective To investigate serum connective tissue growth factor (CTGF) and plasminogen activator inhibitor-1 (PAI-1) levels in patients with primary immunoglobulin A nephropathy (PIgAN). Methods The serum CTGF and PAI-1 levels were assayed by using ELISA in 50 patients with PIgAN and 30 health subjects (control group). According to the Lee's pathological grading, the cases were divided into mild group and severe group. The receiver operating characteristic (ROC) curve was used to analyze prognostic values of serum CTGF and PAI-1 levels for PIgAN patients. Results The serum levels of CTGF and PAI-1 in patients with PIgAN were higher than those in control group (both P＜0.05). Compared with the mild group, the serum levels of CTGF and PAI-1 in severe group were increased significantly (all P＜0.05). ROC curve showed the AUC of CTGF and PAI-1 was 0.844 (95%CI: 0.736～0.995, P＜0.05) and 0.900 (95%CI: 0.811～0.988, P＜0.05) respectively; whereas the AUC of combined CTGF and PAI-1 was 0.927 (95%CI:0.856～0.998, P＜0.05). Conclusion The combination of serum CTGF and PAI-1 levels may improve the evaluation of disease severity for patients with PIgAN. |
