目的探讨肝动脉插管化疗栓塞(TACE)治疗对肝癌组织HIF3α表达的影响及其与患者临床病理特征及新生微血管密度(MVD)的相关性。方法采用免疫组织化学染色(IHC)检测128例原发性肝细胞癌患者实施TACE(72例)后二期手术切除的肝癌组织及未实施TACE(56例)手术切除的肝癌组织的HIF3α及CD34蛋白表达,根据HIF3α阳性染色计分,CD34阳性染色计数MVD,登记相关肝细胞癌患者的临床病理参数。采用t检验比较实施与未实施TACE患者肝癌组织HIF3α蛋白表达差异;采用Pearson相关检验分析实施TACE治疗患者肝癌组织中HIF3α蛋白表达水平与临床病理特征及MVD的相关性。结果与未实施TACE患者的癌旁组织相比,实施TACE患者癌旁组织HIF3α蛋白表达显著增强(t=-22.789,P<0.01),未实施TACE者癌旁组织中心区呈低表达(低表达者占95.83%);实施TACE者肝癌组织栓塞周围组织HIF3α蛋白表达与肿瘤直径(r=0.250,P<0.05)、原发灶浸润深度(r=0.275,P<0.05)、TNM分期(r=0.430,P<0.01)及MVD(r=0.799,P<0.01)均呈正相关,与其他临床病理参数无明显相关。结论TACE治疗可增强肝脏栓塞周围组织HIF3α蛋白表达,HIF3α表达与TACE治疗后肝脏栓塞区周围血管重建及肿瘤进展有密切关联,HIF3α可作为肝癌TACE治疗后血管重建及肿瘤进展的一种标志分子。 |

Objective To investigate the correlation of HIF3α expression with histopathological features and microvessel
density (MVD) in patients with hepatocellular carcinoma after transcatheter arterial chemoembolization (TACE). Methods One hundred and twenty eight patients with primary hepatocellular carcinoma were enrolled, including 72 cases undergoing TACE (TACE group) and 56 cases without TACE (non-TACE group). The expressions of HIF3α and CD34 (marker for MVD) proteins in
hepatocellular carcinoma tissues were detected by immunohistochemistry (IHC), HIF3α positive staining score and MVD were
calculated. The difference between groups was analyzed by t-test; the correlation of HIF3α protein expression with histopathological parameters and MVD was analyzed by Pearson correlation test. Results Compared to non-TACE liver tissue, HIF3α protein expressed in TACE hepatocellular carcinoma tissue was significantly increased (t=-22.789, P<0.01), while the central area of most non-TACE carcinoma (95.83% ) showed low HIF3α expression. HIF3α protein expression was positively correlated with tumor size (r=0.250, P<0.05), primary tumor invasion depth (r=0.275, P<0.05), TNM staging(r=0.430, P<0.01) and MVD(r=0.799, P<0.01) in the area of surrounding TACE embolism. Conclusion The expression of HIF3α protein in hepatocellular carcinoma is increased after TACE treatment, and is correlated with angiogenesis and treatment response. |
