Nd:YAG激光后囊膜切开术对人眼波前像差的影响

摘要:
目的研究Nd:YAG激光后囊膜切开术对人眼波前像差的影响。方法纳入接受Nd:YAG激光后囊膜切开术患者29例(35眼),治疗前后予以常规裂隙灯显微镜检查,并检测最佳矫正视力、眼压及3、6mm瞳孔下全眼、角膜像差。并对治疗前后的最佳矫正视力、像差进行比较。结果患者治疗前后最佳矫正视力均有提高,差异有统计学意义(P<0.01)。治疗前后眼压变化无统计学差异。3、6mm瞳孔直径下的角膜像差治疗前后的差异均无统计学意义。6mm瞳孔直径下全眼的Z4(离焦)、Z12(球差)术后低于术前,差异均有统计学意义(均P<0.05);全眼的Z5(散光)术后高于术前,差异有统计学意义(P<0.05)。3mm瞳孔直径下全眼的Z5(散光)、Z13(二级散光)术后高于术前,差异均有统计学意义(均P<0.05)。结论Nd:YAG激光后囊膜切开术后视力显著提高;6mm瞳孔直径下全眼球差的像差减少,6mm瞳孔直径下全眼散光以及3mm瞳孔直径下的散光、二级散光像差增大。

Abstract:
Objective To study the effects of Nd:YAG laser posterior capsulotomy on wavefront aberration of human eyes. Methods Twenty nine patients receiving Nd:YAG laser posterior capsulotomy (35 eyes) were enrolled in this study. The routine slit lamp examination and examination of corrected visual acuity, intraocular pressure, and wavefront aberration with 3mm and 6mm pupil diameter were performed before and after operation. Results Best corrected visual acuity was improved significantly postoperatively(P<0.01). There were no significant differences in the intraocular pressure and cornea wavefront aberration before and after operation. In eyes with 6mm pupil diameter, postoperative Z4(defocus) and Z12(spherical) were lower, Z5 (astigmatism) was higher than those before operation (P<0.05), in eyes with 3mm pupil diameter the postoperative Z5 (astigmatism) and Z13 (secondary astigmatism) were higher than those before operation (P<0.05). Conclusion Nd: YAG laser posterior capsulotomy can improve the visual acuity, decrease defocus(with 6mm pupil diameter) and spherical wavefront aberration(both 3, 6mm pupil diameter) and increase astigmatism and secondary astigmatism(3mm pupil diameter).
