目的 探讨血清脂联素水平及腺苷酸活化蛋白激酶(AMP)Kα2 基因多态性(rs2796516)与多囊卵巢综合征 (PCOS)易感性的相关性。 方法 以住院确诊为 PCOS 的 80 例患者为 PCOS 组,选择同期与其种族、年龄、BMI、空腹血糖、空腹胰 岛素等因素相匹配的健康体检妇女为对照组,检测并比较临床及生化指标,同时分析 AMPK α2 基因多态性与 PCOS 易感性的关 系。 结果 PCOS 组血清脂联素水平为(25.99±9.59)mg/L,明显低于对照组(14.99±7.23)mg/L(P<0.01)。PCOS 组 AMPK α2 基 因多态性 G 等位基因频率为 67.5%,低于对照组的 86.5%(P<0.01)。多因素 logistic 回归分析提示促黄体素(OR=2.390,95%CI: 1.536~3.719)、GA/AA 基因型(OR=3.613,95%CI:1.283~10.174)为正相关因素,脂联素(OR=0.778,95%CI:0.697~0.869)、促卵泡激 素(OR=0.186,95%CI:0.087~0.397) 为负相关因素。logistic 回归模型判断 PCOS 易感性的能力一致率为 82.78%,ACU 为 0.875 (95%CI:0.824~0.926),拟和能力中等;最大 Youden 指数为 0.645,对应的预测概率为 0.489,确定该点为最佳诊断点,其灵敏度为0.775,特异度为 0.870。 结论 血清脂联素水平、AMPK α2 基因多态性(rs2796516)与 PCOS 易感性存在一定的相关性,本研究建 立的遗传 - 激素模型对 PCOS 易感性的评估能力中等。 |

Objective To investigate the correlation of AMPKα2 gene (rs2796516) polymorphism and serum adiponectin level with polycystic ovary syndrome. Methods Eighty patients with PCOS and 100 race, age, BMI, fasting plasma glucose and insulin levels-matched healthy subjects (control group) were enrolled in the study. AMPKα2 gene (rs2796516) polymorphism was examined with polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and sequence-based typing, serum adiponectin levels were measured with ELISA. Results Serum adiponectin level was significantly higher in PCOS group than that in controls (25.99±9.59mg/L vs 14.99±7.23mg/L, P<0.01). The frequency of G allele of AMPKα2 gene (rs2796516) in PCOS group and control group were 67.5% and 86.5%, respectively (P<0.05). Logistic analysis showed that LH (OR=2.390, 95% CI:1.536~3.719), GA/AA genotype (OR=3.613, 95% CI:1.283~10.174) were positively correlated with PCOS, while adiponectin (OR=0.778, 95% CI:0.697~0.869), FSH (OR=0.186, 95% CI: 0.087~0.397) were negatively correlated with POCS. The logistic regression genetic-hormone model showed the area under ROC curve for predicting PCOS was 0.875 (95 %CI:0.824~0.926).Conclusion The level of serum adiponectin and AMPKα2 gene polymorphism(rs2796516) are correlated with PCOS susceptibility, and the genetic-hormone model has moderate ability in assessment of PCOS susceptibility. |
