目的探讨婴幼儿早期(出生后20周内)全身运动(GMs)不安运动阶段质量评估对婴幼儿神经系统疾病的预测价值。方法利用数据库检索到2015年12月前发表的相关文献,共有16篇文献纳入研究并进行Meta分析。结果16篇文献QUADAS评分≥10的有8篇,临床特征等信息差异均无统计学意义(P>0.05)。GMs不安运动阶段质量评估对神经系统发育不良结局(包括脑性瘫痪)的预测分析显示,灵敏度、特异度、阳性似然比(PLR)、阴性似然比(NLR)和诊断比值比(DOR)分别为0.78、0.93、11.26、0.24和55.43;SROC曲线表明灵敏度和特异度最佳结合点的Q值为0.8522,AUC值为0.9190。GMs不安运动阶段质量评估对脑性瘫痪的预测分析显示,灵敏度、特异度、PLR、NLR和DOR分别为0.91、0.94、12.91、0.12和133.66,SROC曲线表明灵敏度和特异度最佳结合点的Q值为0.9185,AUC值为0.9692。结论GMs不安运动阶段质量评估是预测婴幼儿神经系统疾病的一种有效方法,但不推荐单独使用。 |

Qualitative assessment of general movements fidgety period in prediction of neurological disorders of early infants: a meta-analysis |

Objective To review the qualitative assessment of general movements (GMs) fidgety period in predicting the neurological disorders of early infants. Methods Using database to search relevant studies published up to December 2015, and 16 studies were selected and meta-analysis was performed. Results The QUADAS scores of 8 studies ≥10 in all 16 studies. there were no significant differences in clinical characteristics among included studies (P>0.05). The results showed that the sensitivity, specificity, PLR, NLR, DOR of GMs fidgety periods in predicting abnormal neurological development outcome (including cerebral palsy) were 0.78, 0.93, 11.26, 0.24, 55.43, respectively. The SROC curve showed that the sensitivity and specificity of the best combination of Q was 0.852 2 with a AUC value of 0.919 0. Predictive analysis in GMs fidgety periods, the results show that The sensitivity, specificity PLR, NLR, DOR for cerebral palsy were 0.91, 0.94, 12.91, 0.12, 133.66, respectively. The SROC curve showed that the sensitivity and specificity of the best combination of Q for cerebral palsy was 0.9185 with a AUC value of 0.9692. Conclusion Qualitative assessment of general movements fidgety period is an effective method to predict
neurological disorders in early infants, but is not recommended to be used alone. |
