不同分娩方式对二胎妊娠结局的影响

摘要:
目的探讨不同分娩方式对二胎妊娠结局的影响。方法收集规范产检并分娩的二胎孕妇4824例。按照前次分娩方式分为前次阴道分娩组3324例和前次剖宫产组1500例两组。比较两组孕妇在二胎分娩时分娩方式、妊娠合并症及并发症发生情况。结果前次剖宫产组本次分娩时剖宫产率为99.60%,高于前次阴道分娩组的54.51%,两组比较差异有统计学意义(P<0.01)。两组子痫前期、子宫破裂发生率比较差异均无统计学意义(均P>0.05);但前次剖宫产组妊娠期糖尿病、前置胎盘、胎盘粘连(植入)、胎盘早剥、产后出血的发生率均高于前次阴道分娩组,差异均有统计学意义(均P<0.05)。结论不同分娩方式对后续的妊娠结局会造成影响,临床应更好地把握剖宫产指征,降低剖宫产率,保障孕产妇健康。

Effect of different delivery methods on outcomes of the second pregnancy

Abstract:
Objective To investigate the effect of different delivery methods on the outcomes of the second pregnancy. Methods Total 4, 824 parous women with second pregnancy were enrolled in the study, including 3 324 cases whose first child was delivered by vaginal route (vaginal delivery group) and 1 500 cases whose first child was delivered by cesarean section (cesarean section group). The delivery method of the second child, the pregnancy complications and comorbidities were compared between two groups. Results The cesarean section rate of the second child in cesarean section group was significantly higher than that of vaginal delivery group (99.60% vs 54.51%, P <0.01). There were no significant difference in incidence of preeclampsia and uterine rupture between two groups (all P >0.05). However, the incidence of gestational diabetes, placenta previa, placenta adhesion (embedded), placental abruption, postpartum hemorrhage in cesarean section group was significantly higher than that in vaginal delivery group (all P <0.05). Conclusion The previous delivery method would affect subsequent pregnancy outcomes, indicating that clinicians should strictly follow the indications to decrease unnecessary cesarean delivery.
