2%羟丙基甲基纤维素在翼状胬肉术中应用对术后干眼症状及泪膜恢复的影响

摘要:
目的探讨2%羟丙基甲基纤维素(HPMC)在翼状胬肉切除术中应用对术后早期干眼症状和泪膜恢复的影响。方法采用随机数字表法将58例原发性翼状胬肉患者分为试验组(28例)和对照组(30例),分别于术前1d及术后1、10和30d对每位受试者依次进行眼表疾病指数(OSDI)问卷、自觉症状评价、泪膜破裂时间(TBUT)、ShirmerⅠ试验(STI)和角膜荧光素染色(FL)评估检查。结果两组患者不同时点OSDI评分值、自觉症状评分及STI值比较差异均无统计学意义(均P>0.05)。术后1和10d试验组TBUT值明显高于对照组,差异均有统计学意义(均P<0.05)。术后1d对照组FL值明显高于试验组,差异有统计学意义(P<0.05)。结论翼状胬肉切除术中应用2%HPMC保持角膜湿润能改善术后早期眼表状态,利于术后早期泪膜功能的恢复,并促进角膜上皮的修复。

Intraoperative administration of 2% hydroxypropyl methyl cellulose improves dry eye symptoms and promotes tear film recovery after pterygium surgery

Abstract:
Objective To investigate the effect of 2% hydroxypropyl methyl cellulose (HPMC) during pterygium surgery on postoperative dry eye symptoms and tear film recovery at early period. Methods Fifty eight patients with primary pterygium were randomized divided into study group (n=28) and control group (n=30), patients in study group were administrated with 2% HPMC intraoperatively, and those in control group received normal saline instead. Ocular surface disease index (OSDI) questionnaire, subjective symptom questionnaire, tear film break-up time (TBUT), Schirmer testing I (STI) and fluorescein staining (FL) were sequentially evaluated preoperatively and at day 1, day 10 and day 30 postoperatively. Results There were no statistical difference in OSDI, subjective symptom and STI value, between the two groups(all P >0.05). The TBUT values at day 1 and day 10 postoperative in study group was significantly higher than those in control group (both P<0.05). The FL value of control group at day 1 after operation was significantly higher than that of the study group (P<0.05). Conclusion Administration of 2% HPMC during pterygium surgery to keep the corneal moist can improve the ocular surface status at early period, and is beneficial for early postoperative recovery of tear film function, and promoting the repair of corneal epithelium.
