目的了解桐庐县居民高尿酸血症(HUA)的流行病学特点及其与慢性肾脏病(CKD)的关系。方法调查2011至2016年(共6年)在桐庐县第一人民医院体检的10~97岁居民共55456人次,分析居民HUA与CKD的患病情况、CKD危险因素及HUA居民的CKD进展情况。结果2011至2015年桐庐县居民HUA患病率呈逐年上升趋势,2016年有所下降;CKD患病率每年基本持平。居民HUA年平均患病率为27.55%,男性高于女性(38.20%vs12.79%,P<0.05);CKD年平均患病率为8.77%,男性亦高于女性(10.91%vs5.81%,P<0.05)。60岁以上男性居民HUA、CKD年平均患病率均高于女性(均P<0.05)。男性、年龄、尿酸、LDL-C、尿素氮等是CKD的危险因素,而女性、白蛋白、HDL-C、空腹血糖、Hb等是CKD的保护因素。HUA居民的CKD进展速度明显快于血尿酸正常居民(P<0.05)。结论桐庐县居民HUA的流行病学特征与国内其它地区类似,HUA居民的CKD进展速度明显快于血尿酸正常居民。 |

Epidemiological features of hyperuricemia in Tonglu county and its relationship with chronic renal disease |

Abstract: |
Objective To analyze the epidemiological features of hyperuricemia (HUA) in Tonglu county and its relationship with chronic kidney disease (CKD). Methods A total of 55,456 local residents aged 10-97 years underwent health check-up in Tonglu county First People's Hospital from 2011 to 2016. The prevalence of HUA and CKD, the risk factors of CKD and the progression of CKD in residents with HUA were analyzed. Results The prevalence of HUA in Tonglu county was increasing from 2011 to 2015, and the prevalence of CKD was relatively stable. The average prevalence of HUA was 27.55%, and the prevalence of males was higher than that of females (38.20% vs 12.79%, P<0.05). The average prevalence of CKD was 8.77% , and the prevalence of males was also higher than that of females (10.91% vs 5.81%, P<0.05). The average prevalence of HUA and CKD in male residents over 60 years was higher than that in female (P<0.05). Male sex, age, higher uric acid, LDL-C, urea nitrogen levels were risk factors of CKD, while female sex, higher albumin, HDL-C, fasting blood glucose, hemoglobin levels were protective
factors of CKD. The progression of CKD in HUA patients were significantly faster than those of non-HUA patients (P <0.05). Conclusion The epidemiological characteristics of HUA in Tonglu county are similar to those in other regions of China. The progression of CKD in HUA patients is faster than that of non-HUA patients. |
