丁苯酞联合阿替普酶对急性缺血性脑卒中患者神经保护作用及血清炎性标志物的影响

摘要:
目的观察丁苯酞联合阿替普酶对急性缺血性脑卒中患者神经保护作用及血清炎性标志物的影响。方法回顾性分析2015年1月至2016年12月分别采用丁苯酞57例、阿替普酶54例和丁苯酞、阿替普酶联合应用49例的急性缺血性脑卒中患者的临床资料,在治疗前,治疗开始后7、14、30、90d采用脑卒中量表(NIHSS)及改良型Rankin量表(mRS)评分,评估3组患者的神经功能,测量脑梗死病灶体积,检测血清IL-6,TNF-α和超敏C反应蛋白(hs-CRP)水平,同时记录治疗期间的不良反应。结果3组患者治疗后第7、14、30、90天的NIHSS评分、mRS评分及IL-6、TNF-α、hs-CRP等炎性因子水平均较治疗前明显下降,脑梗死体积明显减小,治疗后联合组各时点上述观察指标均优于丁苯酞组和阿替普酶组,差异均有统计学意义(均P<0.05)。3组患者在治疗期间的不良反应发生率差异无统计学意义(P>0.05)。结论丁苯酞联合阿替普酶治疗急性缺血性脑卒中,可有效保护患者的神经功能,明显降低患者的炎症反应程度,安全性高,值得临床推广应用。

Abstract:
Objective To investigate the effect of butylphthalide combined with alteplase on neuroprotection and serum inflammatory markers in patients with acute ischemic stroke. Methods One hundred and sixty patients with acute ischemic stroke admitted in our hospital from January 2015 to December 2016 were assigned to receive butylphthalide (butylphthalide group, n=57), alteplase (alteplase group, n=54) or butylphthalide combined with alteplase (combination group, n=49). The neural functions were evaluated with National Institute of Health Stroke Scale (NIHSS) and modified Rankin scale (mRS) ; the volume of brain infarct was measured; the serum IL-6, TNF-α, hs-CRP levels were determined before and 7, 14, 30 and 90d after treatment. And the adverse reactions during treatment were recorded. Results At 7, 14, 30 and 90d after the treatment, the NIHSS and mRS scores, serum IL-6, TNF-α, hs-CRP were significantly decreased, and the infarct volume was significantly reduced compared with those of before treatment in all three groups. And the above indicators of combination group were significantly better than those of other two groups at all time points (P<0.05). There was no significant difference in the incidence of adverse reactions among the 3 groups during the treatment (P >0.05). Conclusion Butylphthalide combined with alteplase can more effectively reduce the inflammatory response and protect neurological function for patients with acute ischemic stroke, compared to the single drug treatment.
