目的观察教育视频用于子宫全切除术后焦虑状态的临床效果,明确子宫全切除术后患者出现焦虑状态的原因。方法选择行腹腔镜下子宫全切除术且术后出现焦虑症状的患者158例,采用随机数字表法分为视频组(观看科普视频)53例、手册组(阅读科普手册)53例和对照组(不进行任何干预)52例,比较术后1、3个月的干预效果。结果术后1、3个月视频组和手册组患者的子宫相关知识正确率均显著高于对照组(均P<0.01),且术后3个月视频组患者的子宫相关知识正确率显著高于手册组。术后1、3个月视频组和手册组患者汉密尔顿焦虑量表评分<14分的例数较对照组显著增加(均P<0.05),且视频组的干预效果明显优于手册组(均P<0.05)。逐步logistic回归分析显示术后1、3个月子宫相关功能的正确观念和绝经是影响术后焦虑状态的独立因素(均P<0.05)。结论视频教育的有效性明显优于传统文本教育,对子宫功能的错误观念可能是导致患者子宫全切除术后出现焦虑状态的主要原因。 |

Effect of intervention with educational video on post-hysterectomy anxiety |

Abstract: |
Objective To assess the effect of intervention with educational video on anxiety status in patients after hysterectomy. Methods One hundred and fifty eight patients with anxiety status after hysterectomy were enrolled in the study. Patients were randomly assigned to watch education video (video group), read brochure (brochure group) for intervention or receive no intervention (control group). The knowledge about uterine function and hysterectomy was tested and the anxiety status was ass- essed with Hamilton Anxiety Scale (HAMA) 1 and3 months after operation. Results After the intervention, the correct rate of un- derstanding uterine function and hysterectomy was significantly higher in video group and brochure group than that in control group
at 1 and3 months postoperatively (all P<0.01); the knowledge level in video group at 3 months postoperatively was signifi- cantly
higher than that in brochure group (P<0.01). The number of patient with HAMA score less than 14 points was significantly higher in video group and brochure group than that in control group at 1 and 3 months postoperatively (all P<0.05); and the interuention effect of video group was better than that of brochure group (both P<0.05). Stepwise logistic regression analysis showed that the correctly understanding uterine function and menopause were independent factors of post-hysterectomy anxiety (all P<0.05). Conclusion
The effectiveness of intervention with educational video is superior to the traditional text reading. The study also shows that misunderstanding of the uterine functions may be the cause of post-hysterectomy anxiety. |
