目的探讨参麦注射液对冠心病早期胃癌患者内镜黏膜下剥离术(ESD)后心功能的影响。方法筛选合并有冠心病的早期胃癌患者共80例,随机分为观察组和对照组,各40例。对照组患者给予ESD治疗。观察组患者于ESD前2周给予参麦注射液静脉滴注,100ml/次,1次/d,术后继续治疗2周。比较两组患者术前术后心功能,包括舒张早期/舒张晚期最大血流速度(E/A)和左心室射血分数(LVEF)、血清肌钙蛋白I(cTnI)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)、活性氧(ROS)、总超氧化物歧化酶(SOD)、丙二醛(MDA)和总抗氧化能力(T-AOC)水平。结果术后1d和术后2周,观察组患者E/A和LVEF均高于对照组(均P<0.05)。术后1d和术后2周,观察组血清cTnI、CK、CK-MB、ROS及MDA水平均低于对照组(均P<0.05),SOD和T-AOC均高于对照组(均P<0.05)。结论参麦注射液对冠心病早期胃癌患者ESD术后心功能起保护作用,改善患者的氧化应激损伤可能是其作用机制之一。 |

Objective To investigate the effect of injection on heart function in coronary heart disease (CHD)
patients with early gastric cancer undergoing endoscopic submucosal dissection (ESD). Methods Sixty CHD patients with early gastric cancer undergoing ESD were randomly divided into control group and study group with 30 cases in each group. Patients in control group received conventional care and patients in study group received additional intraveous infusion of
injection 100ml q.d. from 1d before ESD to 2 weeks after ESD. Heart function including E/A , LVEF was compared between two groups. Serum levels of troponin-I(cTn I), creatine kinase isoenzyme (CK-MB), creatine kinase (CK), reactive oxygen species (ROS), malondialdehyde (MDA), superoxide dismutase (SOD), and total antioxidant capacity (T-AOC) were detected and
compared in two groups. Results The E/A and LVEF in study group were significantly higher than those in control group at all time points (P<0.05). Serum levels of cTnI, CK, CK-MB, ROS and MDA in study group were significantly lower, while SOD and T-AOC were significantly higher than those in control group at all time points (P<0.05). Conclusion injection can improve oxidative stress level and protect cardiac function in CHD patients with early gastric cancer undergoing endoscopic
submucosal dissection. |
