| 目的探讨绍兴市社区精神分裂症患者治疗依从性的影响因素。方法收集2012年6至9月结合精神病防治管理网络分层抽样现况调查选取绍兴市社区服药的精神分裂症患者600例。采用自制的《病人基本情况调查表》记录患者的一般资料，采用自制的《病人治疗依从性调查表》调查统计患者治疗的依从性情况，使用logistic回归分析患者的病程、职业、文化程度、医疗费用、住院时间、服药知识的掌握情况、不良反应量表（TESS）评分、复诊情况、简明精神病量表（BPRS）评分等因素对精神分裂症患者治疗依从性的影响，并分析患者治疗依从性差的原因。结果600例精神分裂症患者中治疗依从性好的312例，占52%，治疗依从性差的288例，占48%。单因素分析结果显示，患者的病程、职业、文化程度、医疗费用、住院时间、服药知识的掌握情况、TESS评分、复诊情况、BPRS评分等因素对绍兴市社区精神分裂症患者治疗依从性有影响，差异均有统计学意义（均P＜0.05）。服药知识的掌握情况、TESS评分、复诊情况和BPRS评分是影响精神分裂症患者依从性的危险因素。患者治疗依从性差的原因主要是认为已经痊愈，不需服药；长期服药，没有信心以及认为药物的疗效不佳。结论影响绍兴市社区精神分裂症患者治疗依从性的因素主要有服药知识的掌握情况、TESS评分和BPRS评分，应加强患者健康知识的教育，完善患者的随访制度，简化患者的治疗方案，提高患者的治疗依从性。 |

| Objective To investigate the influencing factors of treatment compliance in community schizophrenia patients in Shaoxing city. Methods During June 2012 to September 2012, 600 schizophrenia patients in the community of Shaoxing city were selected by stratified sampling to participate in this questionnaire survey on the treatment compliance. Logistic regression was used to analyze the association of treatment compliance with the disease duration, occupation and education levels, medical expenses, hospitalization time, knowledge of medication, TESS score, re-visits, BPRS score and other factors; and the reasons for poor treatment compliance were also analyzed. Results Of the 600 patients with schizophrenia, there were 312 patients (52%) with good treatment compliance and 288 patients (48%) with poor compliance. Univariate analysis showed that the patient treatment compliance was affected by disease course, occupation, education, medical expenses, hospitalization time, knowledge
of medication, TESS score, re-visits, BPRS score (all P ＜0.05). Multivariate logistic analysis showed that the knowledge of
medication, TESS score, re-visits and the BPRS score were risk factors for treatment compliance in schizophrenic patients. The main reasons for poor treatment compliance were that "disease has been cured and no need to take medicine", "long-term medication, no confidence" and "the poor efficacy of drugs". Conclusion The factors influencing the treatment compliance of schizophrenic patients in Shaoxing City are mainly knowledge of medication, TESS score and BPRS score. In order to improve the patient treatment compliance, it is necessary to strengthen the knowledge education for patients, to perfect the patient follow-up system and simplify the treatment plan. |
