小气道功能指标在儿童哮喘缓解期病情评估中的意义

摘要:
目的探讨小气道功能指标在儿童哮喘缓解期病情评估中的意义。方法对98例哮喘儿童进行急性发作期、缓解期肺功能测定,比较小气道功能的变化以及缓解期小气道功能障碍组与肺功能正常组的临床特征。结果哮喘缓解期第一秒最大呼气量占预计值百分比(FEV1%)、用力呼气峰流速占预计值百分比(PEF%)、25%肺活量时的呼气流速占预计值百分比(FEF25%)均高于发作期,差异均有统计学意义(均P<0.05)。缓解期小气道功能障碍组和肺功能正常组在年龄、规律吸入激素疗程、过敏性鼻炎病史、特异性皮炎病史、哮喘家族史等方面比较差异均无统计学意义(均P>0.05)。小气道功能障碍组患儿的病程长于肺功能正常组患儿,哮喘严重程度重于肺功能正常组患儿,差异均有统计学意义(均P<0.05)。小气道功能障碍组哮喘重度风险是功能正常组的4.127倍(95%CI:1.755~9.707,P<0.05)。结论缓解期哮喘患儿小气道功能指标仍有异常,与病程及病情严重程度一致,可用于缓解期哮喘病情的评估。

Small airway function in evaluation of stable asthmatic children

Abstract:
Objective To assess the application of small airway function tests in stable asthmatic children. Methods Ninety-eight asthmatic children were recruited. The parameters of pulmonary function tests in acute exacerbation and clinical remission were compared.Among children in clinical remission, there were 58 cases with normal lung function, and 40 cases with small airway dysfunction. The clinical features were compared between the two groups. Results The levels of percentage of the forced expiratory volume in 1 second(FEV1%), the maximum peak expiratory flow(PEF%), the forced expiratory flow at 25% (FEF25%) were significantly higher in clinical remission than those in acute exacerbation(all P<0.05). There were no significant differences in age, duration of treatment, atopic dermatitis, allergic rhinitis, family history of asthma between normal lung function group and small airway dysfunction group(all P >0.05). The severity of asthma, the duration of wheezing history were significantly higher in the small airway dysfunction group than those in the normal lung function group(both P<0.05). The risk of severe asthma in the small airway dysfunction group was 4.127 times higher than that in the normal lung function group (95%CI:1.755~9.707, P< 0.05). Conclusion The asthmatic children in clinical remission still have peripheral obstruction. Small airway function indexes are of value in assessment of asthma status in stable asthmatic children.
