全身营养状况对经皮椎体后凸成型术后椎体再骨折发生的影响

摘要:
目的探讨全身营养状况与经皮椎体后凸成形术(PKP)后椎体再发骨折的关系。方法选取2014年1月至2015年12月因骨质疏松性椎体骨折行PKP的患者143例。测量BMI、上臂围、小腿围、血红蛋白、白蛋白、微型营养评估简表(MNA-SF),分析营养状况对再骨折的影响。结果143例中,47例有椎体再骨折。再骨折组BMI、上臂围、白蛋白、血红蛋白、MNA-SF评分较无新发骨折组更低(P<0.05或0.01)。多元回归分析显示白蛋白水平(OR=0.93,P=0.01),MNA-SF评分(OR=0.65,P=0.003)与再骨折的发生显著相关。ROC预测曲线显示白蛋白和MNA-SF预测再骨折发生的灵敏度分别是62.4%和73.0%,特异度分别是72.1%和67.2%。结论PKP术患者全身营养状况较差,术后再骨折发生率较高。白蛋白和MNA-SF评分与PKP术后再骨折发生显著相关。

Effect of nutritional status on the incidence of new vertebral fractures after percutaneous kyphoplasty

Abstract:
Objective To investigate the relationship between nutritional status and the occurrence of new vertebral fractures (VFs) after percutaneous kyphoplasty (PKP). Methods One hundred and forty three patients (102 women, mean age 74.7 years) with osteoporotic vertebral fractures treated with PKP from January 2014 to December 2015 were recruited in the study, including 47 cases (32.9% ) having new VF (VF group) and ninety six without new VF (non-VF group). The nutritional parameters, including BMI, upper arm circumference, calf circumference, hemoglobin, albumin were measured, and the nutritional status was also assessed with mini nutritional assessment short form (MNA-SF) at the time of admission. The nutritional status was compared between two groups. Regression analysis, t-test, and receiver operating curve were used for statistical analysis. Results Compared to non-VF group, patients in VF group had lower BMI, less upper arm circumference, lower albumin and hemoglobin levels, and lower MNA-SF scores (P<0.05). Multiple regression analysis showed that albumin levels (OR=0.93, P=0.01) and MNA-SF score (OR=0.65, P=0.003) were associated with the occurrence of new VFs. Taking 35.1 and 10 as the cutoff value of albumin and MNA-SF for predicting occurrence of new VF, the sensitivity were 64.0% and 73.0%, and the specificity were 72.1% and 67.2% , respectively. Conclusion The occurrence of new VF is increased with the decreasing nutritional status. Albumin level and MNA-SF score may be used for predicting the occurrence of new VFs in patients with vertebral fractures after PKP.
