NICU与普通新生儿病房患儿AABR听力筛查结果与听力损伤危险因素分析

摘要:
目的观察新生儿重症监护病房(NICU)与普通新生儿病房患儿自动听性脑干反应(AABR)听力筛查结果,分析其听力损伤的危险因素。方法回顾性分析NICU(1106例)与普通新生儿病房(1757例)患儿临床资料、AABR检查结果及脑干听觉诱发电位(ABR)检查最终诊断结果。比较NICU与普通新生儿病房患儿AABR检查结果、先天性感音神经性耳聋(SNHL)发生情况,分析患儿发生SNHL的危险因素。结果NICU患儿AABR检查未通过率高于普通新生儿病房患儿(6.78%vs3.93%,P<0.05),SNHL发病率亦高于普通新生儿病房患儿(3.98%vs1.88%,P<0.05)。宫内窘迫、1minAPGAR评分降低、新生儿高胆红素血症、机械通气≥5d、颅内感染、脓毒症均为患儿发生SNHL的独立危险因素(均P<0.05)。结论NICU患儿听力损伤发生率高于普通新生儿病房患儿,对存在听力损伤危险因素的新生儿如AABR检查未通过需高度重视并密切随访,以早期干预降低致残率。

Hearing screening for newborns in neonatal ICU and neonatal ward

Abstract:
Objective To evaluate the result of hearing screening with automated auditory brainstem response (AABR) for newborns in neonatal intensive care unit (NICU) and for those in neonatal wards. Methods The results of hearing screening with AABR and final auditory brainstem response (ABR)of 1 106 newborns from NICU and 1 757 newborns from neonatal ward in Ningbo Women and Children Hospital were retrospectively analyzed. The results of AABR and final diagnosis of sensorineural hearing loss(SNHL) were compared between two groups of newborns, and the risk factors of SNHL were analyzed. Results The positive rate of AABR screening and SNHLin newborns of NICU were significantly higher than those in neonatal ward (6.78% vs 3.93%, P<0.05; 3.98% vs 1.88%, P<0.05, respectively). Intrauterine distress, low APGAR scores at 1 min, hyperbilirubinemia, mechanical ventilation ≥5d, intracranial infection and sepsis were independent risk factors of SHNL. Conclusion The incidence of SNHL in NICU is higher than that in neonatal ward. The newborns with positive AABR hearing screening and the risk factors of SHNL need to be closely monitored for hearing status during childhood.
