目的对比自动乳腺全容积扫查系统(ABVS)与手持式超声(HHUS)作为第二眼超声检查对MRI检出的高危乳腺病灶的诊断价值。方法采用前瞻性研究方法,选取MRI检出乳腺高危病灶的患者116例,共132个高危病灶,均行ABVS和HHUS作为第二眼超声检查。以病理学检查结果为标准,比较ABVS、HHUS在诊断乳腺良恶性病灶上的差异,以及两者对不同MRI影像特征病灶的检出情况。结果ABVS的乳腺高危病灶检出率高于HHUS(76.5%vs69.7%,P<0.05);而两种超声检查在诊断乳腺良恶性病灶上的差异比较无统计学意义(P>0.05),ABVS、HHUS对MRI影像特征上不同大小、内部强化类型、时间-信号增强曲线类型的病灶检出率比较差异均无统计学意义(均P>0.05),而对不同增强类型的病灶检出率比较差异有统计学意义(P<0.05),两种超声检查对非肿块强化型病灶的检出率均低于对肿块型病灶的检出率。结论对于乳腺高危病灶,ABVS作为第二眼超声检查比HHUS检出率更高,但两者对非肿块强化型的病灶检出率均不高。 |

ABVS and HHUS as second-look ultrasound examination in diagnosis of high-risk breast lesions on MRI |

Objective To compare the diagnostic value of automated breast volume scanner (ABVS) and hand-held ultrasound (HHUS) as second-look ultrasound examinations for high-risk breast lesions on MRI. Methods One hundred and sixteen patients with 132 high-risk lesions on breast MRI were enrolled in the study. The patients underwent HHUS and ABVS examinations as second-look ultrasound modalities. Based on pathologic findings, the value of ABVS and HHUS in differential diagnosis of benign and malignant breast lesions was compared; and the detection rates of two modalities for lesions with
different characteristics on MRI were analyzed. Results The detection rate of ABVS for high-risk lesions was higher than that of HHUS (76.5% vs. 69.7%, P<0.05), but there was no significant difference in differential diagnosis of benign and malignant breast lesions. The detection rate of two ultrasound methods was no statistically different for lesions with different size, internal enhancement patterns and TIC curves on MRI (P >0.05), but there was significant difference in detection of lesions with different enhancement types on MRI between two methods. The detection rate of non-mass lesions was lower than that of mass-type
lesions by both methods. Conclusion ABVS is more effective than HHUS in detection of high-risk lesions as the second-look ultrasound examination. However, both techniques have limitations in detecting non-mass enhancement. |
