目的系统评价金属基质蛋白酶2(MMP-2)与食管癌临床特征的关系。方法利用PubMed、EMbase、CNKI、维普和万方数据库检索关于MMP-2与食管癌临床特征关系的文献,并进行质量评价。应用Revman5.3软件进行Meta分析。结果共纳入28篇文献。Meta分析结果:MMP-2在食管癌组织中的表达明显高于正常对照组织(OR=10.90,95%CI:7.23~16.44,P<0.05);MMP-2与淋巴结转移(OR=4.38,95%CI:3.38~5.48,P<0.05)、临床TNM分期(OR=2.59,95%CI:1.46~4.58,P<0.05)、肿瘤浸润深度(OR=5.00,95%CI:3.66~6.83,P<0.05)、肿瘤直径(OR=1.70,95%CI:1.16~2.49,P<0.05)、3年生存期(HR=1.46,95%CI:1.16~1.84,P<0.05)均相关。而MMP-2表达阳性与阴性的食管癌组织分化程度比较,差异无统计学意义(OR=1.51,95%CI:0.77~2.97,P>0.05)。结论MMP-2与食管癌患者淋巴结转移、临床TNM分期、肿瘤浸润深度、肿瘤直径等有关,与分化程度的关系尚不确定;提示MMP-2在食管癌的发生、发展过程中可能起着重要作用。 |

Clinical significance of MMP-2 expression in esophageal carcinoma: a meta-analysis |

Abstract: |
Objective To investigate the expression and clinical significance of MMP-2 in esophageal carcinoma (EC). Methods PubMed, EMbase, CNKI, VIP and Wanfang databases were searched for relevant literature on the relationship between MMP-2 expression and clinical features of esophageal cancer. The quality of the included studies was evaluated according to the Newcastle-Ottawa Scale (NOS). Statistical analysis was performed using Revman 5.3 software. Results This meta-analysis includes a total of 28 articles. Meta-analysis showed that the expression of MMP-2 in esophageal cancer was higher than that in normal control group (OR=10.90, 95%CI:7.23-16.44, P<0.05). The positive expression of MMP-2 was associated with the depth of invasion, lymph node metastasis (OR=4.38, 95%CI:3.38-5.48, P<0.05), clinical stage (OR=2.59, 95%CI:1.46-4.58, P<0.05), depth of invasion (OR=5.00, 95%CI:3.66-6.83, P<0.05), tumor size (OR=1.70, 95%CI:1.16~2.49, P<0.05) and overall survival
(HR=1.46, 95% CI:1.16-1.84, P <0.05) in esophageal cancer, but not associated with the differentiation degree of esophageal cancer (OR=1.51, 95%CI: 0.77-2.97, P=0.23). Conclusion The positive expression of MMP-2 is associated with the depth of in- vasion, lymph node metastasis, clinical stage and tumor size in esophageal cancer, indicating that MMP-2 may play an important role in the occurrence and development of esophageal cancer. |
