		     | 目的探讨可溶性fms样酪氨酸激酶1(sFlt-1)对急性胰腺炎(AP)早期诊断及严重程度评估的价值。方法70例AP患者按严重程度分为轻度组43例、中重度组27例;另选同期健康体检者30例为对照组。行血常规、淀粉酶(AMY)、脂肪酶、肝肾功能、空腹血糖(FBG)等检测,采用ELISA法检测不同时点(急诊时及入院第1、2、7天)血清sFlt-1水平。结果中重度组患者血AMY、脂肪酶、TC、FBG、超敏C反应蛋白(hs-CRP)、WBC水平均明显高于轻度组(均P<0.05)。中重度组血清sFlt-1水平明显高于轻度组、对照组(均P<0.015),而轻度组又高于对照组(P<0.05)。入院第1、2天,AP患者血清sFlt-1水平逐渐上升,第7天较急诊时明显下降(P<0.05)。血清sFlt-1与AMY呈正相关(r=0.360,P<0.05)。血清sFlt-1与AMY联合检测的AUC明显高于单个指标检测(均P<0.05),其灵敏度、特异度均优于单个指标检测(均P<0.05)。结论血清sFlt-1水平在一定程度上能反映AP严重程度,对AP早期诊断及严重程度评估具有一定的参考价值。 |

                | Objective To investigate the value of soluble fms-like tyrosine kinase 1 ( sFlt-1 ) in diagnosis of acute pancreatitis ( AP). Methods Seventy patients with AP were enrolled and divided into mild group ( MAP group, n=43 ) and moderately/severe group (MSAP group, n=27), 30 healthy subjects were simultaneously enrolled as the control group (n=30). Blood routine, amylase (AMY), lipase, liver and kidney function, fasting blood glucose (FBG) and other indicators were measured. Serum sFlt-1 was detected by enzyme-linked immunosorbent assay( ELISA ) at emergency admission and 1d, 2d, 7d after admission. Results The blood AMY, lipase, TC, FBG, high-sensitivity C-reactive protein (hs-CRP) and WBC levels were significantly higher in the MSAP group than those in the MAP group (P<0.05). The level of serum sFlt-1 in MSAP group was significantly higher than that in MAP group and control group (P<0.05) and serum sFlt-1 levels in MAP group were significantly higher than those in control group (P<0.05). Serum sFlt-1 levels in AP patients gradually increased on the 1st and 2nd day after admission, but  decreased  significantly  on the 7th day, and  significantly  lower than that  at  admission(P<0.05). There  was a  
positive correlation between serum sFlt-1 and blood AMY levels (r=0.360, P<0.05). ROC curve analysis showed that the area under the curve(AUC) of the combined detection of AMY and sFlt-1 was significantly higher than sFlt-1, AMY alone (P<0.05). The sensitivity and specificity of the combined detection were better than single index (P<0.05). Conclusion  Serum sFlt-1 levels can reflect the severity of the disease in some extent. It has a certain clinical reference value in diagnosis and severity assess- ment of acute pancreatitis. |
