生理通气辅助下CT靶扫描对肺局灶性磨玻璃密度阴影的显示价值

摘要:
目的探讨生理通气辅助下CT靶扫描对肺局灶性磨玻璃密度阴影(fGGO)的显示价值。方法选取常规肺部低剂量螺旋CT筛查时发现的连续50例fGGO患者为研究对象。在确定病灶位置后,直接利用低剂量筛查数据进行回顾性512矩阵靶重建(B方案),并对病灶进行常规512矩阵高分辨率扫描(A方案)和生理通气辅助下靶扫描(C方案);比较3种扫描方案的参数。结果总体诊断效能及病灶细节分辨能力的最佳方案为C方案。A、B、C方案各感兴趣区CT值所在层面肺野平均CT值比较,差异均有统计学意义(均P<0.05);病灶密度、病灶肺门侧、病灶层平均密度CT值比较,差异亦均有统计学意义(均P<0.05);病灶相对密度CT值比较,差异无统计学意义(P>0.05)。结论生理通气辅助下CT靶扫描技术有助于提高fGGO检出率,使细节显示更清晰。

Value of physiological ventilation-assisted target CT scanning for lung focal ground glass opacity
Abstract:
Objective To assess the value of CT scanning assisted by physiologic ventilation in the display of lung focal ground-glass density opacity (fGGO). Methods Fifty consecutive patients with lung fGGO detected by conventional low-dose spiral CT screening were enrolled as study objects. After the location of the lesions was determined, retrospective 512 matrix target reconstruction (plan B) was performed using low-dose screening data, and routine 512 matrix high-resolution scanning (plan A) and physiologic ventilation-assisted target scanning (plan C) were performed for the lesions. The parameters of three scanning schemes were compared. Results Plan C was the best solution for overall diagnostic efficacy and focus detail resolution. There were statistically significant differences in the average CT values of the lung field at each interest area level among plan A, B and C (all P<0.05). The CT values of lesion density, lung portal and lesion layer mean density were significantly different among three plans (all P<0.05). There was no significant difference in CT values of relative density of lesions among three plans(P >0.05). Conclusion CT scanning assisted by physiological ventilation can improve the detection rate of fGGO and make the details clearer.
