稳定期COPD患者血清镁、钙和维生素D水平与生活质量评分的相关性研究

摘要:
目的探讨稳定期慢性阻塞性肺病(COPD)患者体内血清镁、钙和维生素D水平与生活质量评分之间的相关性。方法连续性纳入COPD患者110例,检测血清镁、钙和25羟维生素D3[25(OH)D3]水平,并进行COPD评估测试(CAT评分)、呼吸困难分级(mMRC分级)以及BODE指数。结果低镁血症组BODE指数[(3.52±1.04)分vs(2.42±0.87)分,P<0.05]、mMRC分级[(2.55±0.33)级vs(2.12±0.25)级,P<0.05]和CAT评分[(15.32±3.01)分vs(10.42±2.91)分,P<0.05]均明显高于血镁正常组,而血清钙和25(OH)D3水平差异均无统计学意义(均P>0.05)。低钙血症组mMRC分级[(2.76±0.31)级vs(2.02±0.26)级,P<0.05]高于血钙正常组,血清钙和25(OH)D3则低于血钙正常组(P<0.05或0.01),BODE指数和CAT评分差异均无统计学意义(均P>0.05)。低镁低钙组BODE指数(3.98±0.77vs3.02±0.98,P<0.05)和CAT评分(15.88±3.89vs12.71±2.85,P<0.05)均高于单纯低钙血症组,而低镁低钙组患者FEV1/FVC明显低于单纯低钙血症组患者(58.05±8.58%vs64.82±6.24%,P<0.05)。血清镁与mMRC分级(r=-0.670,P<0.05)、BODE指数(r=-0.635,P<0.05)和CAT评分(r=-0.661,P<0.05)均呈负相关。结论稳定期COPD患者血清镁离子、钙离子和维生素D水平与生活质量评分密切相关,值得关注。

Association between serum magnesium, calcium, vitamin D levels and quality of life in patients with stable COPD

Abstract:
Objective To investigate the association between serum magnesium, calcium, vitamin D levels and quality of life in patients with stable chronic obstructive pulmonary disease (COPD). Methods One-hundred and ten stable COPD pa- tients were enrolled in this study. The serum levels of magnesium, calcium and vitamin D were measured. COPD assessment test (CAT) score, modified Medical Research Council (mMRC) dyspnea score and a multidimensional grading system BODE index (body mass index, degree of airflow obstruction, dyspnea and exercise capacity) were used for evaluating the quality of life of patients. Results The CAT scores (15.32±3.01 vs 10.42±2.91, P<0.05), mMRC score (2.55±0.33 vs 2.12±0.25, P<0.05) and BODE index (3.52±1.04 vs 2.42±0.87, P<0.05) in patients with magnesium deficiency were higher than those in patients with magnesium sufficiency, however, there were no significant differences in serum levels of calcium and vitamin D3 between two groups (P >0.05). The mMRC scores (2.76±0.31 vs 2.02±0.26, P<0.05) in patients with calcium deficiency were higher than those in patients with calcium sufficiency, however, there were no significant contents of serum calcium, vitamin D3, BODE and CAT scores between two group (P >0.05). The BODE (3.98±0.77 vs 3.02±0.98, P<0.05) and mMRC scores (15.88±3.89 vs 12.71±2.85, P<0.05) in patients with both calcium and magnesium deficiency were higher than those in patients with calcium deficiency. The levels of FEV1/FVC (58.05±8.58% vs 64.82±6.24%, P<0.05) in patients with both calcium and magnesium defi- ciency were lower than those in patients with calcium deficiency. Correlation analysis showed serum magnesium levels were negatively associated with mMRC, BODE index and CAT scores. Conclusion Serum magnesium, calcium and vitamin D are re- lated to the quality of life in patients with stable COPD.
