腹膜透析对慢性肾脏病患者脂联素水平、微炎症及营养状态的影响

摘要:
目的探讨腹膜透析对慢性肾脏病(CKD)患者血浆脂联素(ADPN)水平、微炎症及营养状态的影响。方法选取CKD患者62例,按患者采用的透析方式不同分为观察组(腹膜透析)和对照组(血液透析),各31例。采用ELISA法检测患者透析前、透析1、3、6个月后血浆ADPN、C反应蛋白(CRP)和IL-6水平;采用营养不良-炎症评分(MIS)评估患者透析6个月后的营养状态;采用肾脏疾病生存质量简表评估患者透析6个月后生活质量。结果随着透析时间的推移,两组患者ADPN、CRP和IL-6水平均呈上升趋势(均P<0.05)。两组患者透析前ADPN、CRP和IL-6水平比较差异均无统计学意义(均P>0.05);观察组患者透析后各时点ADPN、CRP和IL-6水平均低于对照组(均P<0.05)。透析6个月后,观察组患者营养不良率低于对照组(P<0.05),生活质量各维度评分均高于对照组(均P<0.05)。结论相较于血液透析,腹膜透析不仅可改善CKD患者微炎症和营养状态,还可提高生活质量,具有一定的优势。

Micro-inflammation and nutritional status of patients with chronic kidney disease undergoing peritoneal dialysis

Abstract:
Objective To investigate the micro-inflammation and nutritional status in patients with chronic kidney disease (CKD) undergoing peritoneal dialysis. Methods Sixty two patients with CKD attending in our hospital from September 2011 to August 2016 were randomly divided into two groups with 31 in each group. Patients in the control group received hemodialysis, while patients in study group received peritoneal dialysis. Serum adiponectin (ADPN), CRP and IL-6 levels were measured by enzyme linked immunosorbent assay (ELISA) after first dialysis, and 1 months, 3 months and 6 months after dialysis. The nutritional status of patients was evaluated by malnutrition-inflammation score (MIS). The quality of life of patients was evaluated by the Kidney Disease Quality of Life-Short Form (KDQOL-SF). Results There were no significant differences in serum ADPN, CRP and IL-6 before dialysis between two groups (P >0.05). After dialysis treatment, the serum ADPN, CRP and IL-6 showed an upward trends in both groups with the time prolonging (P<0.05), however, the levels in study group were significantly lower than those in control group at each time points (P <0.05). After 6 months of dialysis, the malnutrition rate in study group was significantly lower than that in control group (P<0.05). The quality of live scores in study group were significantly higher than those in control group (P<0.05). Conclusion Compared with hemodialysis, peritoneal dialysis can more significantly reduce malnutrition and improve the nutritional status and quality of live in CKD patients, which is associated with the reduction of inflam- matory factors and ADPN levels.
