		     | 目的通过检测女性系统性红斑狼疮(SLE)患者血清25-羟基维生素D(25-OH-D)水平,并分析其与其他实验室指标的相关性,探讨该类患者25-OH-D水平变化的临床意义。方法收集75例女性SLE患者和70例女性健康对照者的血清,采用电化学发光免疫检测25-OH-D水平;通过Pearson相关分析25-OH-D与系统性红斑狼疮活动度评分(SLEDAI评分)、补体3(C3)、补体4(C4)、血清钙(Ca)、血清磷(P)、碱性磷酸酶(ALP)、甲状旁腺素(PTH)、24h尿蛋白定量以及治疗药物(激素、羟氯喹)的相关性。结果(1)女性SLE患者血清25-OH-D水平明显低于对照组;(2)女性SLE患者血清25-OH-D水平与SLEDAI、激素用量及时间、24h尿蛋白定量水平均呈负相关(r=-0.61、-0.68、-0.74、-0.37,均P<0.05),与Ca呈正相关(r=0.247,P=0.042);而与C3、C4、P、ALP、PTH水平及使用羟氯喹无相关性;回归分析显示SLE中25-OH-D缺乏组(<10ng/ml)较25-OH-D正常组(≥30ng/ml)相比,狼疮活动是非活动发生的5.60倍(OR=5.6,95%CI:1.146~27.37,P=0.033)。结论女性SLE患者中血清25-OH-D水平明显降低,25-OH-D水平与SLE疾病活动、使用激素及肾损害相关,25-OH-D可能在女性SLE发病中起着重要作用。 |

                | Objective To investigate serum 25-hydroxyl vitamin D  (25-OH-D) levels in female patients with  systemic lupus erythematosus (SLE) and its clinical significance. Methods Seventy five female SLE patients (SLE group) and 70 cases healthy females (control group) were enrolled in the study. Serum 25-OH-D levels were detected with ECL method. The correlation of 25-OH-D levels with SLE disease activity index (SLEDAI), complement C3, complement C4, calcium (Ca), phosphorus(P), alkaline phosphatase(ALP), parathyroid hormone(PTH), 24 h urinary protein(24Pro) and administration of hormone, hydroxychloroquine were analyzed with Pearson analysis.  Results  Serum 25-OH-D levels in the female SLE patients were
significantly lower than those in control group. Serum 25-OH-D was negatively correlated with SLEDAI (r =-0.61, P <0.01),
dosage of hormone (r=- 0.68, P = 0.02), duration of prednisone (r=-0.74, P=0.04), 24 pro (r=-0.37, P<0.05), and positively correlated with Ca (r=0.247, P=0.042). No correlation was found between 25-OH-D and C3, C4, ALP, PTH, hydroxychloroquine. Logistic regression analysis indicated that compared to normal 25-OH-D level (≥30ng/ml), 25-OH-D <10 ng/ml was associated with increased SLEDAI(OR=5.6, 95%CI:1.146-27.37, P=0.033). Conclusion Serum levels of 25-OH-D in female SLE patients are significantly decreased, which are closely corelated with disease activity, administration of hormones and renal damage. |
