血友病A型患者焦虑抑郁情绪与生活质量的研究

摘要:
目的研究血友病A型患者疾病的严重程度与焦虑、抑郁情绪的关系,以及对患者生活质量的影响。方法将33例血友病A型患者按疾病严重程度分为重型组(FⅧ浓度<1%)8例和轻/中型组(FⅧ浓度≥1%~25%)25例,采用广泛性焦虑量表(GAD-7)、抑郁症筛查量表(PHQ-9)评估患者的焦虑和抑郁情绪,采用健康调查问卷(SF-36)评估患者的生活质量,比较不同严重程度患者焦虑抑郁的状态以及对生活质量的影响。结果重型组GAD-7及PHQ-9量表分值均高于轻/中型组,差异均有统计学意义(均P<0.05);SF-36量表中生理职能的分数两组间差异有统计学意义(P<0.05)。结论重型血友病A型患者的焦虑、抑郁程度显著高于轻/中型患者;重型血友病A型患者与轻/中型血友病A型患者之间生活质量的差异主要体现在生理职能的变化上。

Abstract:
Objective To study the anxiety, depression and quality of life in patients with hemophilia type A. Methods Thirty three patients with hemophilia type A aged 18~60 year were enrolled in the study from January to September 2016. The patients were classified as severe (FⅧ level<1%, n=8) and mild/moderate (FⅧ level 1%~25%, n=25) groups. The anxiety and depression of patients were assessed with Generalized Anxiety Disorder (GAD-7) scale and Patient Health Questionnaire (PHQ-9), the quality of life was assessed with Medical Outcome Study Short from Health Survey (SF-36). According to the integral change statistical analysis, the anxiety and depression status was compared between patients with different degree of severity and its influence to the quality of life was analyzed. Results The GAD-7 and PHQ-9 scores were higher in severe group than those in light/moderate group(P<0.05). Role-physical (RP) of SF - 36 scale score was statistically significant different between the two groups (P <0.05). Conclusion Patients with severe hemophilia type A have higher degree of anxiety and depression than those with mild/moderate disease. The lower quality of life of patients with severe disease mainly reflects on the changes of role-physical.
