献血人群中HCV感染与HLA-II类基因的相关性研究

摘要:
目的人类白细胞抗原(HLA)在机体抗病毒免疫中起重要作用,探讨献血人群中丙型肝炎病毒(HCV)感染与HLA-II类基因的相关性。方法选择2011年10月至2013年10月在浙江省血液中心献血的人员中HCVRNA阳性的样本45例,同时以健康人群为对照,其中HLA-DRB1位点对照样本8333例,HLA-DRB1位点对照样本72例,用PCR-SBT方法测定献血者HLA-II类基因中DRB1和DQB1等位基因情况。结果HLA-DRB1基因的DRB1*01:02(P=0.0394,OR=23.4,95%CI=2.89~189.1),DRB1*13:01(P=0.0095,OR=5.60,95%CI:1.74~18.00);HLA-DQB1基因的DQB1*03:03(P=0.0402,OR=0.45,95%CI:0.21~0.94),DQB1*06:01(P=0.0456,OR=2.47,95%CI:1.05~5.83),这些基因位点的多态性在HCVRNA阳性献血者中的频率与健康对照人群比较有统计学差异。结论在献血人群中HCV感染与HLA-II类基因中部分多态性位点有相关性。

Association of HLA-II alleles with hepatitis C virus infection in blood donors

Abstract:
Objective To investigate the association of HLA-II alleles with hepatitis C virus (HCV) infection in Zhejiang blood donors. Methods Forty five HCV positive cases were detected by blood nucleotide screening among blood donors in Zhejiang Provincial Blood Center from October 2011 to October 2013. HLA typing was performed with polymerase chain reaction analysis with sequence-based typing(PCR-SBT) in 45 HCV infected blood donors; the healthy population was used for reference, including 8333 cases for HLA-DRB1 and 72 case for HLA-DQB1. Results The polymorphisms of HLA-II alleles DRB1*01:02 (OR=23.4, 95%CI: 2.89-189.1, P=0.0394), DRB1*13:01 (OR=5.60, 95%CI: 1.74-18.00, P=0.0095), DQB1*03:03 (OR=0.45, 95%CI: 0.21-0.94, P=0.0402,), DQB1*06:01 (OR=2.47, 95% CI: 1.05-5.83, P=0.0456) were significantly associated with HCV infection among blood donors. Conclusion Our data suggest that certain HLA-II alleles are associated with HCV infection as a host genetic factor among the Zhejiang blood donors.
