目的探讨丁苯酞对血管性痴呆患者脑循环动力学及血管内皮功能的影响。方法71例血管性痴呆患者随机分为观察组(丁苯酞治疗)35例和对照组(常规治疗)36例,规范治疗12周。比较两组患者治疗前后脑循环动力学参数(平均血流速、平均血流量、脑血管床特性阻抗、外周阻力),外周血内皮微粒(EMPs)、内皮祖细胞(EPCs)及血清基质细胞衍生因子-1α(SDF-1α)、血管内皮生长因子(VEGF)水平,临床疗效及不良反应发生情况等。结果治疗12周,观察组脑循环平均血流速、平均血流量均高于对照组(均P<0.05),脑血管床特性阻抗、外周阻力均低于对照组(均P<0.05);总有效率高于对照组(P<0.05)。与治疗前比较,观察组治疗12周时外周血CD31+/CD42b-、CD144+水平均明显下降(均P<0.05),CD62E+水平明显升高(P<0.05),循环EPCs、血清SDF-1α及VEGF水平均明显上升(均P<0.05);治疗12周,观察组与对照组上述指标比较差异均有统计学意义(均P<0.05)。结论丁苯酞能改善脑循环动力学,调控血管内皮功能相关因子的表达,对血管性痴呆的疗效明显,值得在临床推广。 |

Effects of butylphthalide on cerebral circulation dynamics and vascular endothelial function in patients with vascular dementia |

Objective To investigate the effects of butylphthalide on the cerebral circulation dynamics and vascular endothelial function in patients with vascular dementia (VaD). Methods Seventy one patients with VaD were randomly assigned to receive conventional treatment (control group, n=36) or butylphthalide therapy (study group, n=35) for 12 weeks. The changes of kinetic parameters of the cerebral circulation and the clinical effects of the two groups were compared. The plasma endothelial microparticles (EMPs) and endothelial progenitor cells (EPCs), serum stromal cell derived factor-1α (SDF-1α) and vascular endothelial growth factor (VEGF) were detected in two groups before and after treatment. Results After treatment, the average blood flow velocity and mean blood flow in the study group were significantly higher than those in the control group (P<0.05). The characteristic impedance and peripheral resistance of cerebral vascular bed were significantly lower than those of the control group (P<0.05). The total effective rate of the study group was significantly higher than that of the control group (P<0.05). Compared with before treatment, the expression of CD31+/CD42b- and CD144+ in the peripheral blood of the study group were decreased and the expression of CD62E+ was increased after treatment (P<0.05), the expression levels of circulating EPCs, serum SDF-1α and VEGF were increased (P<0.05). Compared with the control group, there was a significant difference (P< 0.05). to vascular endothelial function in patients with VaD. |
