超声引导下区域神经阻滞用于胸腔镜手术的研究进展

摘要:
视频辅助胸腔镜手术凭借其创伤小、恢复快的特点,已成为胸科手术的主要选择。然而,术后疼痛的管理仍是临床亟待解决的问题。近年来,超声引导下区域阻滞镇痛技术因其显著的镇痛效果,在胸科手术中的应用备受关注。本文系统综述了超声引导区域神经阻滞在胸腔镜手术中的应用现状和研究进展,研究表明,区域神经阻滞技术可有效减少围手术期镇痛药物用量,减轻术后急性疼痛,提高患者舒适度,并降低术后并发症风险。然而,其在术后慢性疼痛、免疫功能调节、癌症复发及术后认知功能障碍等方面的影响亟需进一步深入研究和验证。未来研究应聚焦于区域神经阻滞技术的安全性、个体化考虑以及长期效果评估,以优化胸外科手术的镇痛管理策略,推动临床麻醉技术的发展和创新。

The Progress of Research on Ultrasound-Guided Regional Nerve Blockade for Video-Assisted Thoracoscopic Surgery

Abstract:
Video-assisted thoracoscopic surgery (VATS) has become the preferred choice for thoracic surgery due to its minimal invasiveness and rapid recovery characteristics. However, postoperative pain management remains a significant clinical challenge. In recent years, ultrasound-guided regional nerve block analgesia has garnered considerable attention for its remarkable analgesic effects in thoracic surgery. This review systematically summarizes the current application and research progress of ultrasound-guided regional nerve block techniques in VATS. The findings indicate that regional nerve block techniques can effectively reduce the use of perioperative analgesics, alleviate acute postoperative pain, improve patient comfort, and reduce the risk of postoperative complications. However, the impact of regional nerve blocks on chronic postoperative pain, immune function modulation, cancer recurrence, and postoperative cognitive dysfunction requires further in-depth study and validation. Future research should focus on the safety, individualized considerations, and long-term effects of regional nerve block techniques to optimize analgesic management strategies for thoracic surgery and promote the development and innovation of clinical anesthesia technologies.
